Director of Home Care and Independent Living
General Purpose :
Responsible for the overall operation of all components comprising Judson Home Care. Judson Home Care serves residents and members on our Judson campuses as well in the greater community, providing personalized services and supports. Works to insure cost-efficient, high-quality programs that meet the needs of Judson residents and community members, ensures that program meets Federal and State licensure certification requirements, and helps Judson improve upon and / or develop new programs that will assist residents and community members in taking responsibility for their successful aging experience. Responsible for ensuring that communication with residents and community members is proactive and responsive.
Oversees residents who live in Judson independent living to ensure the maximum health and wellness, autonomy, and quality of living for them [CS1] is maintained. Provides supervision and guidance to associates providing direct care services to Judson independent living residents. Serves as liaison between Judson and the IL community regarding resident needs. Provides leadership and oversight of the nurses serving independent living residents.
Essential Duties and Responsibilities :
Home Care :
- Monitor, assess, and inventory service needs, service options, resource utilization, and revenues and expenses, including any grant funding available to the program.
- Fiscal accountability to budget and home care delivery metrics
- Work closely with the Sales and Marketing team on strategies to increase visibility of home care in the community
- Maintain necessary standards for licensure.
- Participate in appropriate State and National professional organizations and alliances
- Responsible for building team and both appreciative and respectful of diversity challenges and opportunities.
- Provides oversight of the nursing assessments and development of plan of care for home care clients.
- Ensures that the required documentation including developing, implementing and evaluating the care plan, as changes in condition occur and as required.
- Prepared monthly billing and bi-weekly payroll through electronic medical record.
- Oversight of care delivered by the home care aide and makes adjustments to plan of care as needed,
- Effective communication with residents and families. Actively listens to resident and family concerns and identifies solutions.
- Oversight of the smart living professionals.
- Promotes and participates in Quality Improvement initiatives.

The physical demands described here are representative of those that must be met by an associate to successfully perform the essential functions of this job :
Requires good vision, hearing, speech and communication skills with the ability to ambulate, project voice, use the telephone, use the computer and read and write English. In addition, the employee must be able to stand on her / his feet for a period of up to eight (8) hours a day, move around the home without difficulty, transfer a 150-pound person and squat on the floor and rise to standing.

The office-working environment is conducive to be able to computerize essential data, communicate effectively on the telephone and communicate effectively with referrers, caregivers, supervisors, R / Cs and responsible or interested parties. When away from the office, must be within paging and (cell) telephone range to effectively communicate scheduling changes. Also, must have access to updated information (i.e. employee. Supervisor and Resident data) at all times.

Each home or residence (working environment) has a “Home Safety Evaluation” conducted at the admission of each case by the RN Community Care manager to confirm that the home environment is safe and to make recommendations as necessary. Flexibility in work hours may be necessary.
