Purpose:
The Community Based Adult Services program is actively seeking an exceptionally skilled and empathetic Registered Nurse (RN) committed to serving the needs of older adults and individuals with disabilities. As an integral part of a collaborative multidisciplinary team, the RN works closely with social workers, program aides, physical therapists (PT), occupational therapists (OT), speech therapists (ST), and a Dietitian. Their collective efforts focus on assessing and supporting program participants, aiming to enable them to remain in their communities, thereby preventing unnecessary hospitalizations and nursing home placements. The RN plays a central role in coordinating all health-related activities within the program. This involves determining nursing and personal care requirements for each participant, engaging with primary care providers and caregivers, and providing comprehensive health education to staff, participants, and their families
Job Responsibilities:
- Develop, oversee, and implement nursing procedures in accordance with regulations and the guidance of the Center Director and licensing.
- Conduct initial assessments of participants to determine the need for nursing and personal care services, documented, dated, and signed by the RN.
- Develop, implement, and monitor individual care plans for nursing and personal care, specifying short-term and long-term goals.
- Reassess nursing and personal care services, documenting the provisions and reactions of participants.
- Interface with primary physicians for necessary forms and notify them of significant changes in participants' conditions.
- Oversee tuberculosis testing or chest x-rays for all ADHC/ADC participants.
- Collaborate with ADHC Medical Director and/or PMD in the initial assessment and ongoing treatment of participants.
- Monitor vital signs monthly or as needed, noting any changes and reporting to families and primary care physicians.
- Cooperate with research efforts related to nursing components of the program.
- Provide one or more services as determined by assessments and specified in the care plan.
- Monitor, administer, and maintain records of medication as per ADHC certification.
- Develop and oversee a continence training program if beneficial for participants.
- Supervise program aides in line with nursing care plans and PT/OT maintenance program implementation.
- Counsel participants, families, and caregivers on health-related issues.
- Provide health education and counseling based on the plan of care.
- Implement emergency procedures and coordinate transportation during emergencies.
- Conduct training sessions for participants in the self-administration of medications.
- Collaborate with the dietitian to adjust menus for special dietary requirements, such as low-sodium and low-cholesterol diets.
- Bachelor’s Degree in Nursing and licensed by the California Board of Registered Nurses with significant relevant experience.
- CPR/BLS Certificate from American Heart Association.
- Minimum of two years’ experience working in the aging field such as Adult Day Health Care, gerontology treatment program, administrative and teaching experience.
- Knowledge and understanding of problems affecting frail elderly and their families.
- Record keeping and assessment experience.
- Bilingual Ability (Spanish/English) preferred.
- Knowledge of word processing, data base, email, Internet, Electronic Health Record, and spreadsheet software/applications.
