Position Purpose : Under the direction of the Chief Medical Officer and in compliance with the agency’s standardized procedures, the Nurse Practitioner (NP) is responsible for delivering high-quality, patient-centered care to the patients of JWCH, Wesley Health Centers. The NP provides primary and preventive healthcare services, including patient assessments, diagnoses, treatment, and management of acute and chronic conditions. This role requires collaboration with a multidisciplinary team to ensure comprehensive care addressing the medical, social, and emotional needs of patients. Additionally, the Nurse Practitioner actively participates in health promotion, patient education, and disease prevention initiatives to enhance overall community health outcomes.
Principal Responsibilities :
- Conduct physical examinations, identify findings / diagnoses, and implement management / treatment plans.
- Elicit and review patient histories in compliance with clinical standards.
- Diagnose and treat acute health conditions, including illnesses, infections, or injuries.
- Counsel patients on prescribed medications, including potential side effects and interactions.
- Order, perform, and interpret diagnostic tests such as CBCs, EKGs, and x-rays.
- Educate patients on self-management strategies for acute or chronic conditions, customized to their circumstances.
- Ensure informed consent for physical assessments and treatments.
- Provide patient education and information relevant to care and treatment plans.
- Follow up on abnormal laboratory results, ensuring appropriate evaluation and care.
- Seek consultations and make referrals as needed for comprehensive care.
- Stay updated with medical advancements through participation in training sessions, symposiums, and didactic courses.
- Identify issues related to clinic workflows, training, or team cooperation and communicate them to the Chief Medical Officer, Clinic Administrator, or Associate Medical Director.
- Train and educate community health workers and nursing attendants as appropriate.
- Conduct quarterly reviews of patient referrals and follow-ups for abnormal lab values or physical findings.
- Perform other duties as assigned.

Note : If you become a JWCH provider, you may be able to participate in Loan Repayment Program . A majority of our clinics are NHSC approved sites. The National Health Service Corp may be accepting applications Loan Repayment Program awards. The program is open to licensed primary care medical, dental, and mental and behavior health providers who are employed or have accepted an offer of employment at an NHSC-approved site. Awardees receive up to $50,000 in exchange for two years of service if they practice in a HPSA of 14 above (Tier I). Awardees can also receive up to $30,0000 in exchange for two years of service if they practice in a HPSA of 13 and below (Tier II). You will need to contact National Health Service Corp to apply and receive more information. Upon acceptance, you are encouraged to apply to National Health Service Corp for Loan Repayment Program.
