This facility is committed to maintaining excellence in admissions, census management, and resident care coordination.
Position Summary & Responsibilities
The Director of Admissions will oversee all admission-related activities, managing census targets and payer mix while ensuring a seamless resident transition process. Key responsibilities include :
- Responding to admission inquiries from hospital social workers, families, and prospective residents.
- Providing information to interested parties and conducting or arranging facility tours.
- Coordinating the admission process, including verifying transfer details, obtaining orders, and notifying relevant team members.
- Ensuring accurate entry of admission details into the Electronic Medical Records (EMR) system.
- Managing and maintaining waitlists for Skilled Nursing and Assisted Living units.
- Developing relationships with hospital case managers, physicians, and referral sources to generate consistent admissions.
- Collaborating with the Business Office to ensure sound financial practices related to census development and volume targets.
- Participating in Quality Assurance and Performance Improvement (QAPI) meetings and preparing required reports.
- Attending Utilization Review meetings to discuss admissions and discharges.
- Maintaining open communication with hospital case managers regarding bed availability and potential resident concerns.
- Coordinating with hospital personnel on discharge planning and readmission strategies.
- Tracking and managing resident hospitalization and re-admission plans.
- Promoting facility services to area hospitals, senior agencies, physicians, and community organizations.
- Developing and distributing marketing materials and brochures to prospective residents and referral sources.
- Representing the facility professionally in outreach and networking events.
