GENERAL SUMMARY
The purpose of this job is to supervise the provision of medical, nursing, and case management services. Develops and implements quality assurance initiatives. Oversees referral and enrollment processes. Serves as a liaison to community partners. Participates in fundraising and budget administration to meet specified goals. You may be required to work in correctional or community health settings based on the assignment. This position supervises the Nurse-Family Partnership program, which works with first-time moms to provide education and support for healthy pregnancy and infant growth and development.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Supervises the work of assigned personnel, including assigning and reviewing work, providing guidance, and conducting performance evaluations.
- Oversees and develops Care Management Program spending and services.
- Implements staff and community education and outreach initiatives; acts as primary liaison between local and State agencies, including Michigan Department of Health and Human Services, Aging and Adult Services Agency, US Aging Network, and local medical and educational systems.
- Assists in implementing quality assurance initiatives and protocols; participates in quality management activities with contracted providers; ensures adherence to State, Federal, and local guidelines.
- Participates in processes to maintain and/or obtain funding to ensure the sustainability of the Division programs in Kalamazoo County.
- Coordinates with other Public Health program supervisors to provide comprehensive public health services to the local community within the county and program policy guidelines.
- Participates in public health emergency responses.
- Performs related work as required.
MINIMUM QUALIFICATIONS
Required Education and Experience
- Bachelor's degree from an accredited college or university in Nursing; Master's degree preferred.
- 3 to 5 years of related work experience in nursing, social work, human services, public health, maternal child health, working with the aging population, etc.
- Any equivalent combination of training, education, and experience that provides the required skills, knowledge, and abilities
Required Licenses or Certifications
- Registered Nurse license with a Bachelor’s Degree in Nursing (BSN)
