Job Description
Summary:
Responsible for overall accountability of coordination of medical and behavioral health between community care navigation and the primary care setting for patients identified requiring complex care management. Accountability for ensuring that a holistic approach is used and that goals for all dimensions of health; psychosocial, behavioral, emotional and medical are met. Collaborates with hospital staff, medical staff, behavioral health staff, nursing, ancillary services, community based organizations, Health Home staff, Managed Care, any other entity necessary for coordinating care on behalf of a client or family/guardian to ensure that pediatric care is coordinated, with all accompanying documentation and referrals, appropriate community referrals, and completed family/children/adult education and linkages. Responsible for project oversight and oversees daily program activities.
Education And Credentials
Responsibilities:
Bachelor's degree in health care / Community required. Master's degree in health care / Community preferred. CANS-NY 0-5 Year Old required within 30 days of hire. CANS-NY 6-21 Year Old required within 30 days of hire. NYS Driver's License upon hire.
Experience
Other information:
5 years of experience in Supervision required. 3 years of experience in Supervision in healthcare or behavioral health setting required. 2 years of experience in Case Management required.
