Job Summary
Providing support services to consumers attending the CDS, Community Day Service.
Responsibilities and Duties
Manage the overall programming for each consumer assigned to the caseload.
Identify, convene, and direct the interdisciplinary team at the time of each consumer’s rehabilitation plan staffing.
Conduct a survey at the time of each consumer’s rehabilitation plan review to ensure all consumer records are complete, current, and accurate.
Initiate, give direction, and monitor DSP’s in implementing individual consumer goals and objectives.
Monitor progress of each assigned consumer in the CDS programs; assist and give direction to DSP’s on training, revising, and modifying methods to simplify techniques.
Ensure safety standards are being met at CDS sites.
Write, maintain, and update individual service plans (ISPs) for each consumer in CDS settings
Serve as a liaison between residential site, day program, community support programs, family, and guardians.
Qualifications and Skills
Bachelor’s Degree in Psychology, Rehabilitation, Counseling, Special Education, or a related field in Human Services and one or more years case management experience for individuals with developmental disabilities.
