JOB SUMMARY
The Family Navigator is responsible for providing in-home short term case management services to TARC families facing multiple basic need challenges. The Family Navigator will work with TARC families to assess strengths and immediate needs of the family. Short-term goal planning will be completed with families which will include linking families to identified community resources to provide stability.

Case Management Activities
Engage families and establish a trusting relationship that is built on a strengths-based approach, is culturally competent.
- Provide on-going assessment of strengths and needs and identify basic need resources.
- Work with family to develop an individualized family service plan based on assessment of needs.
- Provide services, support and connection to community resources.
- Advocate for the family to other service providers in the community.
- Provide transportation for families when needed.
