Responsible for the coordination and management of daily operations of Social Work programs to facilitate cost effective, timely patient discharge and provision of optimal support services in the community.
QUALIFICATIONS
License / Certification
Required :
Licensed Clinical Social Worker (LCSW)
Kaweah Health approved crisis intervention training within 60 days of hire / transfer.
Experience
Required : Two years of clinical experience in a health care setting.
JOB RESPONSIBILITIES
Essential
Directs and coordinates the activities and programs of the Social Work department.
Responsible for meeting with social workers in the following areas : acute patient care units located in the main campus; Acute Rehabilitation Hospital (inpatient and outpatient units); South Campus, outpatient Dialysis / CAPD units; Home Health; Specialty Clinics.
Coordinates social work representation on Kaweah Health Boards and committees.
Provides education to department, hospital staff, and community on health and social work, and psychosocial aspects of illness.
Resolves complaints from patients, families and community members.
Intervenes in crisis situations, providing needed assessment, counseling and information to patients and their families.
Makes rounds and responds to referrals from physicians and staff providing guidance to necessary social services.
Provides consultation to physicians, hospital staff and community as it relates to patient care needs.
Accumulates information on community resources and links patients, families, and staff to appropriate services.
Provides support and knowledge on medical-social issues through structured community outreach programs.
Participates in program planning. Identifies gaps in services and programs and makes recommendations for meeting community needs.
Integrates social work services with the Kaweah Health's primary functions.
Develops and implements policies and procedures consistent with the Kaweah Health Mission that guide and support the Provision of Services Plan.
Provides supervision to non-licensed staff and can provide billable services (psychosocial evaluations) independent of the Director's supervision.
Supervises social work staff to maintain flow and quality of work to ensure timely accomplishment of workload and adherence to standards of quality.
Determines the qualifications and competence level for department staff. Makes recommendations regarding required staffing levels.
Provides orientation, in-services and continuing education for staff.
Responsible for preparing staff schedules that meet the needs of the patient population and average daily census.
Interviews and hires new employees.
Evaluates staff annually. Completes the performance appraisals within 30 days of the due date.
Identifies performance or education issues through observation, monitoring patient satisfaction outcomes and processes, and promptly communicates concerns to the staff member / s.
Continuously assesses and improves department performance.
Utilizes HR policies and procedures to ensure Kaweah Health employment standards are met. Holds staff accountable if standards are not met.
Recommends requirements for space and other resources. Participates in selecting outside sources for needed services.
Enhances professional growth and maintains awareness of current industry trends, legislation or research which could impact department and the Kaweah Health.

Knowledgeable of growth and development for all patient / family cultural, linguistic, spiritual, gender, and age specific needs.
