The Department of Family Medicine of the Keck School of Medicine of USC, Alhambra Campus, is seeking a Community Health Worker to join and work with a team of experts that are developing and implementing a risk assessment screen and intervention tool for primary care settings. The focus of this work will center around the family care partners/caregivers and the prevention of elder mistreatment in people living with dementia.

The CHW will offer participating at-risk caregivers preventive elder mistreatment resource services. They will promote well-being for the participating at-risk caregivers through individualized care plans and goal setting. The CHW serves as a navigator to link web-based resources to this participant population through a culturally responsive lens to improve outcomes for at-risk caregivers and their families.
 

The applicant must be able to interact comfortably with a variety of clinical staff, organizations, and local community-dwelling caregivers in order to facilitate the coordination of efforts. The CHW will be required to assist with community outreach, participant recruitment, research enrollment, intervention assessment, and data collection.  Some local travel among agencies will be required.

Preferred Education: Bachelor’s Degree in a related field – Spanish Fluency a plus, caregiving for an older adult experience a plus

Preferred Experience: at least 2 years in research interviewing


The hourly rate range for this position is $23.04 – $30.91. Minimum Education: High school or equivalent Minimum Experience: 1 year Minimum Skills: Working knowledge of local maps/freeway systems. experience providing medical/clinical guidance. Experience working with unsheltered and unhoused groups, and/or lived experience with homelessness. Advanced judgment, analytical, and decision-making skills, able to work independently with minimal supervision. Data gathering and analysis skills. Excellent time management skills. Demonstrated interpersonal, negotiation and problem-solving skills. Experience in customer service. Practiced use of discretion and decision-making ability. Excellent written and oral communication skills. Proven ability to build and maintain deep and meaningful relationships with community members and outside agencies. Preferred Education: Related undergraduate study And Specialized/technical training Preferred Certifications: Certifications in CPR, basic life support and/or mental health first aid. Preferred Experience: 2 years Preferred Skills: Demonstrated experience in community healthcare and/or street medicine. A current understanding and appreciation of the cultural vibrancy and historical significance of local communities in Los Angeles and Southern California. Extensive knowledge and/or understanding of numerous cultural issues (e.g., immigration, socioeconomic, LGBTQIA ). Fluent in one or more languages in addition to English (e.g., Spanish, Korean, American Sign Language).
