About this role:
With guidance and support from the Independent Living (IL) Director, the Independent Living Case Manager provides supportive community case management services to program participants. Assists the program participant in planning and implementing services that lead to the development of skills in preparation for their transition to independence. Promote individuals' growth in their areas of interest, decision making, community engagement, and personal accomplishments.
Schedule: 10 am – 6 pm, Sunday - Thursday
We are looking for people who have:
- Bachelor’s Degree in the Human Services field preferred or 5 years’ experience in a related field.
- LSWA/LSW strongly preferred.
- Strong interpersonal, organizational and communication skills.
- Commitment to community-based support to individuals.
- Able to meet required competency level in Microsoft Office software.
- Knowledge of community resources available to assist participants in achieving their goals.
The physical requirements for this role are:
- Extensive travel may be required, including the transportation of youth.
- Ability to sit for extended time periods.
- Frequent telephone use required.
- Frequent computer use required.
In this role, you will get to:
- Work one on one with a program participant and DCF.
- Support the program participant in learning about the resources in their community and how to access them.
- Learn and adhere to the Kennedy-Donovan Center and the Department of Children and Families (DCF) philosophy and mission statements.
- Develop a respectful and trusting rapport with youth.
- Participate in the development of treatment plans.
- Provide ongoing supports related to achievement of Treatment Plan goals and objectives.
- Provide direct case management services to biological/foster families and youth.
- Provide advocacy for individuals as needed.
- Collaborate with internal and external community resources and providers to ensure safe and appropriate placement services.
- Participate in staff meetings, group and individual supervision and case reviews.
- Comply with all KDC, EEC and DCF documentation requirements.
- Participate and co-lead monthly IL trainings held at KDC.
- Provide transportation to youth participants as needed.
- Assist with job search and support engagement in recreational activities.
- Participate in Foster Care Reviews, IEP meetings, and identified meetings as needed.
- Assess individual child/youth needs, family strengths and needs, and develop safe and helping relationships with assigned individuals.
- Assistaing with processing referrals and intakes as directed.
- Perform other job-related responsibilities as assigned.
