Under the direction of the Family and Community Services Manager, this position is responsible for coordinating services to children/families with long-term support needs. Duties include assessment, evaluation, service planning, and provision as well as coordination of all other identified services. A critical component is advocating for the identified needs or wants of individuals.
1. Participate in the assessment and evaluation of all program referrals for CLTS and gather the necessary documentation.
2. Complete CLTS functional screens and re-evaluate functional level of care and eligibility at regular 6-month reviews and as needed with a client needs change.
3. Work with children and their families to develop an Individualized Service Plan (ISP) to address identified needs and ensure that services address person-centered outcomes.
4. Monitor ISP services and needs continually to ensure changes are made as needed.
5. Inform clients and/or families of their rights and responsibilities regarding services, privacy, and grievance procedures.
6. Coordinate services for all program participants including coordination with therapists and other service agencies.
7. Develop effective provider networks to ensure accessibility to all required services.
8. Assist with establishing, maintaining, and updating authorizations with service providers.
9. Calculate, invoice, and monitor parental payments required by the state for program eligibility.
10. Ensure all services provided with funding meet program standard criteria.
11. Develop programs and services to prevent abuse and neglect through supportive family services.
12. Inform all clients and families of how to report suspected abuse and neglect.
13. Prevent unnecessary separation of children from their families through intensive preservation services.
14. Minimize the length of time children remain in foster care through safe and expedient reunification.
15. Help reduce service-related trauma to clients.
16. Assist with transitions out of an institutional setting; assist with relocation to community living settings.
17. Assist with completing financial eligibility forms for Medicaid as well as forms required for other programs such as applying for disability determination and heating assistance.
18. Review billing and expenditures for accuracy and service plan alignment.
19. Complete critical incident reports as needed.
20. Attend school IEP meetings and participate in the discussion of service needs; provide advocacy as needed.
21. Support and assist with transitional service coordination including transition to high school and adult services.
22. Assist in coordinating work-related and vocational services for children when appropriate.
23. Participate in continuing education to enhance case management skills.

5. Basic knowledge of recovery-based principles, mental health-related disorders, substance abuse-related disorders, and trauma-informed care principles is preferred.
Tasks performed are inside and protected from the weather about 75% of the time. Most work is completed in a sedentary position and in a normal office setting. While performing the duties of this job one may be required to sit for a period of up to 2 hours. The employee must be able to receive and enter information using a standard computer operating system. The employee must be able to review and sign documents and records relevant to this position. The employee is required to stand, walk, use hands and fingers, feel, push, pull, reach with arms, hands above and below shoulders, stoop, kneel, and crouch. The employee may occasionally lift or move up to 25 pounds. 25% of the time, functions include travel to and ability to access private homes, schools, hospitals, residential facilities, conference centers for training and other offices.
