We are currently looking for a full-time Case Manager to join our City Residential team!

WORK ENVIRONMENT
The RRP Case Manager works in an office setting. The PRP/COP Case Manager is not expected to be exposed to unusual levels of heat, cold, light, or noise. The position may require repetitive use of computer, keyboard and mouse.
TRAVEL
The PRP/COP/RRP Case Manager is expected to travel locally between the office and client residences and local government offices. The PRP/COP Case Manager is not expected to travel overnight or out of state.

We are currently looking for a full-time Case Manager to join our City Residential team!
What We Need
- Someone who has either experience or education related to the mental health field and an eagerness to grow in the field.
- Someone who has knowledge of the DSM and relevant symptoms of the most common diagnosis within the PRP programs.
- Someone who can demonstrate proficiency in assessments, treatment planning, advocacy skills and case management skills.
- Someone who can demonstrate the ability to express themselves well through writing, interpersonal and oral communication skills.
- Someone who has an understanding and sensitivity to serving culturally diverse and/or special needs population while maintaining patient confidentially.
- The Case Manager is responsible for the comprehensive services offered through on-site and off-site programs. This includes assessment, case management, psycho education, crisis intervention, advocacy, and community liaison.
- The Case Manager is responsible for monitoring completion all documentation such as treatment plans and progress notes by responsible staff.
- The Case Manager is responsible for the submission of treatment plans for authorizations as well as maintenance of charts.
- The Case Manager is responsible for teaching community skills including ADL’s as needed. In addition, The Case Manager is responsible for the training of new employees and the supervision of the program in the event the manager or director is not available.
- The Case Manager is responsible for maintaining their level of skill by completing the required 20 hours per year of training. In addition, each employee is expected to abide by all safety rules and regulations of the department and facility.
- The Case Manager is responsible for carrying out the mission, philosophy of care and policies of Key Point. They also perform other duties as needed to achieve program goals and objectives.
- The Case Manager may be assigned various supervisory roles as determined by the Program Director
- Bachelor’s degree in psychology, sociology, psychiatric rehab, mental health, or a related human services field.
- Valid and active Maryland driver's license (or state of residence)
- Pass a criminal background check, MVA check, and provide a negative drug screen.
- Must have own vehicle.
