What are the responsibilities and job description for the Discharge Case Manager position at Key Recovery and Life Skills Center?
Job Summary:
The Discharge Case manager will assist the Clinical Department and patients, including but not limited to, providing support during the initial admission, up until the preparation for discharge from Key Recovery. The Discharge Case Manager is an integral part of our clinical team by supporting the counselors in providing a trauma informed care environment and supporting the patients in their recovery journey. The discharge case manager will help patients continue any mental health counseling, medical care follows up, medication management, and housing. While preparing for discharge, the case manager will assist the patients to establish clean and sober housing, employment and/or education, access childcare, admit into continued behavioral health services and other services in the community, and establish a clean and sober support system in the community.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Assist with entering data into electronic health records, scheduling any needed appointments in the EHR system, supporting housing needs, and updating other clinical staff with patient needs.
- Support SUD clinicians with the establishment of the SUD treatment plan and developing needs assessment to help support the patient meet their treatment objectives and goals.
- Works with SUD counselors to help establish patient treatment schedules, routines, coordinating external provided services, video calls, court dates, legal representation calls and arranging other resources including transportation and escort.
- Develop a service plan with each patient to determine the resources available to meet the identified needs in a coordinated and integrated manner, and advocate with community service providers to assist the patient in achieving the goals of the service plan.
- Supervises clients out of structured clinical activities.
- Monitors patient progress by verifying patient participation, attendance and responses, and communicating with clinical staff team about the patients’ progress, areas of concern, etc.
- Advocate for needed services for patient and residential children, obtaining additional resources, intervening in emergent situations and crisis, provides overall support to both patients and staff members.
- Maintains client records by reviewing case notes, logging events and recording progress.
- Communicates patient progress by participating in weekly interdisciplinary meetings and evaluations, disseminating results and obstacles to clinical staff.
- May assist with create childcare/child development plan with each parenting patient to include (a) an assessment of the family’s needs, (b) a description of childcare that is offered onsite and/or resources that are available to patient and children in their community, (c) methods to access childcare through state and local agencies, and (d) any additional requirements.
- Provide physical safety information and/or training to patients while in treatment or recovery housing as well as maintain current medical emergency information.
- Develop and maintain program rules and responsibilities for the patients to encourage group compatibility and to assist the patients in learning the skills necessary to progress in their recovery.
- Participate in weekly group meetings with the patients to discuss program management, including patient’s rights and responsibilities and to problem solve any conflicts that arise in group living.
- Prepares patient discharge by reviewing plans with clinical staff, coordinating discharge and postdischarge requirements and coordinating aftercare resources.
- Participates in continuing education, especially in the area of substance use disorder and family services.
- Attends all mandated meetings.
- Other duties as assigned.
MINIMUM QUALIFICATIONS:
EDUCATION AND EXPERIENCE
- High School degree or GED, required.
- 2 years prior case management experience or similar experience required.
- AA Degree in chemical dependency studies or bachelor’s degree preferred.
- Experience working with patients with SUD and cooccurring disabilities.
- Requires a valid Washington state driver’s license and maintain active auto insurance on personal vehicle.
- Computer and EHR literate.

- Ability to maintain confidentiality with sensitive information in accordance with HIPAA, 42 CFR and 45 CFR.
- No history of misuse of alcohol or other drugs for a minimum of one year prior to employment with Key Recovery Center. Employer is a drugfree workplace, and employees will agree to preemployment and reasonable cause drug testing policies.
PHYSICAL DEMANDS:
The duties require sitting, standing, walking, stooping, bending, smiling, kneeling, reaching, pushing, pulling, twisting, ability to grip with hands and fingers. Ability to hear and speak conversationally, fluent in English occasionally lifting up to 25 pounds. Must be able to climb stairs without an elevator.
