Job Description :
We are seeking an experienced Level 1 Medicaid Waiver Provider to join our team. The ideal candidate will have a strong background in providing essential services to individuals with disabilities, seniors, or other eligible populations under the Medicaid Home and Community-Based Services (HCBS) Waiver Program. This role requires a dedicated and compassionate professional who can deliver high-quality support to enhance the independence and quality of life of our clients.
Key Responsibilities :
Provide direct support and personal care services to eligible individuals in compliance with Illinois Medicaid Waiver regulations.
Monitor and document clients’ progress, ensuring all required reports and documentation are accurate and submitted on time.
Communicate effectively with clients, families, case managers, and other healthcare professionals to coordinate services.
Ensure compliance with all state and federal Medicaid regulations, health and safety standards, and ethical guidelines.
Qualifications :
Must be an approved and registered Level 1 Medicaid Waiver Provider in the state of Illinois.
Minimum of 1-2 years of experience providing Medicaid waiver services or related support to individuals with disabilities or senior populations.
Strong knowledge of Illinois Medicaid Waiver policies and procedures.
CPR and First Aid certification (preferred).
Ability to pass a background check and meet all Illinois Department of Healthcare and Family Services (HFS) requirements.
Excellent communication, documentation, and organizational skills.
