Position Title: Program-based LPN

Introductory Period: 90 days
Reports To: School Nurse
Summary/Objective:
The Classroom/Program-based LPN provides health services to strengthen and facilitate the educational process by modifying or removing health-related barriers to learning in the individual students. Promote an optimal level of wellness of students and staff by providing preventative health services to facilitate the students and staff’s optimal physical, mental, emotional and social growth and development.
Essential Functions/Duties:
Provide medical interventions, such as medication administration, care to ill children and first aid to injured students.
Maintain appropriate medical records database.
Work with medical providers and families and to obtain and maintain student health information.
Provide required medical records and information to district nurses.
Work with the Program Supervisor or classroom staff to develop classroom and student safety plans.
Assist in the development of student health care plans.
Interface with family members and providers regarding student medical issues.
Provide resources for and deliver health education to students and staff.
Ensure confidentiality of student health related records and documents.
Participate in team meetings, as appropriate.
Ensure proper infection control procedures and equipment are in place.
