The Director of Health & Wellness oversee and manage the wellness programs at our Assisted Living and Memory Care community. The ideal candidate will be responsible for coordinating and implementing various health and wellness initiatives to promote the overall well-being of our residents.
Responsibilities:
- Develop, implement, and evaluate wellness programs to meet the physical, emotional, and social needs of residents
- Conduct documentation review and ensure compliance with all regulatory standards
- Provide case management for residents requiring specialized care
- Monitor patient progress and adjust care plans as needed
- Collaborate with healthcare professionals to deliver critical care services
- Ensure adherence to HIPAA regulations in all aspects of resident care
- Oversee home health services and coordinate with external healthcare providers
- Manage a team of nurses and healthcare staff effectively
- Maintain accurate medical records using electronic systems
Qualifications:
- Bachelor's degree in Nursing or related field; Master's degree preferred
- Proven experience in nurse management within a healthcare setting
- Strong knowledge of medical records systems and documentation practices
- Familiarity with care plans development and patient monitoring protocols
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a multidisciplinary team environment
