Primary Job Responsibilities:
- Receives referrals via inbound phone call and referral networks
- Ensures that patients and referral sources are properly informed of the agency's services
- Determines agency's ability to accept referrals via location, date, discipline, clinical requirements and patient insurance
- Enters all necessary information into the Electronic Medical Record
- Distributes patient assignments to clinical teams considering patient needs and staff availability and efficiency
- Ensures patient care is initiated within regulatory and agency defined timelines
- Communicates effectively with patients, caregivers, and referral sources regarding care initiation and changes
- Reassigns clinical staff schedules in the event of planned and unplanned time off
- Delivers a 5-star customer experience to all internal and external consumers
- Accepts direction from CEO and VP of Clinical Services
- Maintain confidentiality with regard to patient information
- Performs other duties as assigned
- 3 years' experience with a healthcare organization; preferably in acute care, home health or hospice environment preferred
- Must be able to multi-task in a fast-paced environment and communicate effectively
- Excellent customer service skills and proven ability to work with healthcare professionals, patients and their families responding to time-sensitive needs
- Excellent data entry skills
- Dedicated self-starter who can work both independently as well as collaboratively as part of a team of healthcare professionals
- Proven technology skills and experience using an electronic documentation system
- Proficient in Microsoft computer products including Word, Excel and Outlook
- Proof of COVID-19 Vaccination or an approved Medical or Religious exemption
- Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption
