What are the responsibilities and job description for the Social Worker & Data Coordinator- Bleeding Disorders Program position at KIDZ Medical Services?
SUMMARY: Seeking a full time social worker to support our pediatric patients with bleeding disorders and manages data collection and reporting for various research databases. Our practice is located on the Nicklaus Children's Hospital campus. Open to remote work option 1-2 days per week after on-site training has been completed, at the discretion of the leadership team.
DUTIES AND RESPONSIBILITIES:
Social Work:
- Conducts psychosocial assessments of patients with bleeding disorders, with subsequent diagnosis of any behavioral, emotional and psychological disorders and the effects on medical status, access to care, coping skills, and support systems during contacts with patients
- Cultivates ongoing relationship with patients and families by phone, email and face-to-face contacts
- Provides behavioral and/or mental health counseling and other forms of therapy services to established patients
- In conjunction with other medical and mental health professionals, develops individual service plans (as needed) for patients and families, supporting access to medical care, financial needs, psycho-social counseling and other needs, referring to internal, community service providers and other organizations as appropriate
- Collaborate with clinical staff for all coordination in patient care, including but not limited to, prior authorization and home health care services
- Maintains proper documentation of all patient/family contacts and completed work in both clinic charting system and Bleeding Disorders database(s)
- Provides individual and family counseling to address emotional needs of patients with bleeding disorders and their families
- Assesses uninsured patients’ eligibility for third-party insurance coverage. Facilitate applications for insurance coverage, patient assistance programs, Social Security or forms of public assistance as appropriate. Educate patients about Medicare & Medicaid benefits. Provide education so they can evaluate coverage options.
- Provides education regarding bleeding disorders, offer adherence counseling and reinforce education given by other team members
- Collaborate effectively with program or project teams to successfully manage quality and process improvement related activities.
- Exhibits a high degree of courtesy, tact, and poise when interacting with patients, families, and other healthcare professionals.
Data Coordination:
- Responsible for developing and implementing processes to manage the CDC Community Counts Project, including:
- Post-clinic and submission of Registries.
- Track CDC Specimen Testing for CDC Community Counts Registry and Clinical Inhibitors
- Maintains access to and training on, The American Thrombosis and Hemostasis Network (ATHN) Clinical Manager (CM) database
- Responsible for running queries, generating reports, auditing, and performing data analyses for the bleeding disorders program
- Performs other related duties as assigned by management.
QUALIFICATIONS:
- Master’s Degree in Social Work
- 2 years of experience as a clinical social worker
- Experience working with patients with chronic, lifelong diseases
- Experience managing patients with comorbidities, both physiological and psychological
- Experience assisting with patients with high levels of distress and coping challenges
- Experience in managing access to care issues, such as patients in remote areas and transportation barriers
- Experience with leading patient/family support groups
- Experience in multi-discipline setting
- Able to work collaboratively with multiple health professionals in a busy and complex environment using tact, diplomacy, and discipline.
