Under the direct supervision of the Administrator, the Adult Day Coordinator is responsible for overseeing the daily
operations of the Adult Day Program, providing support to direct care staff, and ensuring the development and
implementation of individual support plans for program participants. This role includes comprehensive case management,
where the coordinator collaborates with care facilities, families, and funding agencies to promote the self-determination
and well-being of clients, tailoring services to meet their specific goals, needs, and abilities.

How will I contribute

• Develop individualized support plans (ISP) and provide case management for clients on caseload, focusing on promoting
  person-centered services according to their goals.
• Collaborate and maintain regular open communication with care facilities, families, and the Central Valley Regional
  Center (CVRC) to maintain service coordination.
• Conduct thorough case management, including monitoring client progress and any changes to support needs.
• Implement Restricted Health Care Plans developed by an RN in collaboration with families/care facilities and CVRC,
  ensuring compliance and proper care for each client for whom a RHCP is required.
• Maintain accurate and up-to-date client information within the database, ensuring all documentation is current and
  accessible.
• Provide guidance and support to staff (DSPs), assisting with the implementation of client support plans.
• Conduct monthly staff training sessions to equip and ensure adherence to best practices.
• Assist in the hiring, training and feedback to new staff members, including conducting evaluations and implementing
  corrective actions and retraining as necessary.
• Troubleshoot and resolve minor staffing or client-related issues, ensuring a positive and supportive environment for both
  staff and clients.
• Monitor and maintain inventory of Adult Day Program supplies, including arts and crafts, personal care supplies, and
  other necessary materials, while adhering to budget restrictions and practicing good stewardship.
• Oversee the cleanliness and maintenance of the facility, reporting repair needs to leadership as necessary.
• Communicate with case management agencies, parents, and other care providers, maintaining positive and productive
  relationships keeping them apprised of client attendance, progress, and any concerns or updates.
• Maintain flexibility in responding to the needs of the program, providing direct services to clients on an intermittent or
  temporary basis as required.
• Conduct electronic charting training for staff, ensuring proper documentation and use of the electronic tracking system
  for client progress and issues.
• Perform other duties as assigned by the Administrator to support the operation and success of the Adult Day Program.

What I bring

• Associate degree in Behavioral Science, Social Work, Nursing, Education or Psychology
• Class C Driving License Required
• Adult First Aid and CPR
• Two years of experience in a similar role within adult day program setting or residential care program for persons with I/DD, including case management responsibilities.
• Strong understanding of care planning, client assessment, case management, and program development for individuals with behavioral health needs. Knowledge of relevant regulations and compliance standards (e.g., Community Care Licensing, Title 22, Title 17).
• Excellent communication and interpersonal skills, with the ability to work effectively with diverse teams, clients, and external stakeholders. Strong organizational and time management skills, with the ability to handle multiple tasks and prioritize effectively. Proficiency in using electronic health record (EHR) systems and other relevant technology. Ability to maintain flexibility and adapt to changing needs within the program.
• Demonstrates the physical and mental resilience necessary to work with clients who may be potentially dangerous to themselves or others, in line with the Company’s Physical and Mental Fitness standards. Capable of passing health screening examinations, including physical, drug, and TB tests. Meets all state personnel licensing requirements. Willingness to work in a culturally integrated workplace and respect for diversity, including differences in race, age, disability, religion, sexual orientation, political beliefs, place of origin, skin color, and other distinguishing characteristics. Commitment to providing high-quality care and support in a culturally diverse environment.
• Preferred: Bachelor’s degree in social work, gerontology, psychology, nursing, or a related human
  services field. Experience working with adults with developmental disabilities.
