We are seeking a dynamic Outreach Supervisor to lead our dedicated Community Outreach Department and help expand access to healthcare for underserved populations. Position Summary:
The Outreach Supervisor coordinates and oversees the daily operations of the Community Outreach team. This includes managing outreach activities, ensuring appropriate program linkage and referrals, maintaining documentation in organization's EMR, and collaborating closely with leadership to ensure the success of department initiatives, including the Mobile Medical Clinic. The role includes both supervisory and direct service responsibilities.
Key Responsibilities:
Supervision & Operations
Supervise outreach staff, manage workflow, performance, and team development
Create and manage team schedules, including timesheets and PTO requests
Train new hires and assist with employee development
Coordinate Mobile Medical Unit coverage, staff assignments, and site locations
Ensure department compliance with HRSA reporting and other grant requirements
Complete and track Strategic Action Plans annually
Report departmental key performance indicators monthly
Community Outreach & Patient Services
Deliver one-on-one and face-to-face health education, surveys, and screenings (BP, glucose checks, etc.)
Coordinate outreach staff coverage based on caseload, geography, and patient health needs
Ensure appropriate referrals and care coordination addressing social determinants (food insecurity, housing, behavioral health, job training, etc.)
Assist with appointment scheduling, patient calls, home visits, and transportation as needed
Ensure outreach staff maintain active caseloads (up to 100 patients) and document accurately in organization's EMR
Community Engagement & Collaboration
Attend and participate in community-based organization meetings and health events
Represent KCHC in community outreach efforts and maintain professional partnerships
Support and lead event setup and breakdown
Participate in professional development and maintain up-to-date knowledge of HIPAA and clinical hygiene standards
Qualifications:
Education:
Associate’s degree or Bachelor’s in Business Administration or related field required
Clinical and administrative support experience preferred
Experience:
3–5 years of community outreach and supervisory experience preferred
Bilingual (Spanish/English) strongly preferred
Equivalent education and experience combinations will be considered
Certifications & Licenses:
CPR certification (or obtained within 6 months of hire)
Valid NC Driver’s License with acceptable driving record
Skills:
Strong communication, leadership, and organizational skills
Proficiency in Microsoft Office (Word, Excel, Outlook), web browsers, and mobile apps
Motivational interviewing and health coaching skills are a plus
Ability to work flexible hours, including evenings and weekends as needed
