Summary of Position

Under the general supervision of AVP-Nursing, the Triage Nurse will be responsible for patient triaging, the nurse phone line, assisting with nurse case management, nursing assessments, collaboration with providers and other departments, and provision of quality patient care in compliance with local, state and federal regulations and accreditation standards. The telephonic triage nurse must adhere to Kintegra Health’s Teleworking Policy.

Experience: Minimum of 3 years Telephonic Triage experience – 1 year minimum of Pediatric Triage experience

Professional Licensure: RN (preferred) or LPN Current North Carolina or Compact State unrestricted license

Certifications: BLS

Responsibilities

  • Telephonic triage of all established patients.
    • Assessment, review of history, review medications, laboratory values, and other elements required for safe disposition of the patient. Disposition is based on acute patient needs.
  • The telephonic triage nurse utilizes assessment skills in conjunction with computerized software embedded into the EMR to assist in clinical decision-making and disposition.
    • The telephonic triage nurse may elevate the disposition based on assessment in comparison to the assisting clinical decision software. However, disposition acuity does not decrease in comparison to assisting clinical decision software.
    • The nurse will consult with medical providers when needed to complete assessment and disposition.
  • Appointments are scheduled based on the acute needs of the patient.
    • Nurses will reach out to identified leadership staff in the clinic to assist in scheduling appointments if timely appointments are not available – making efforts to avoid unnecessary ED or Urgent care referrals.
  • The triage nurse provides care instruction and education for each patient based on clinical needs, timing of appointments, and home care needs specific to the patient's condition, history, and age.
  • Ensures accurate notes of telephonic triage are documented within the EMR.
  • Adheres to privacy, confidentiality, and HIPAA laws and regulations.
  • Serve as a clinical resource and professional role model for other nursing staff.
  • Prioritizes work tasks and services and completes tasks in a timely manner.
  • Provides patient education regarding disease processes, therapies, and healthful behaviors.
  • Participate in staff planning, in-service, and other meetings as needed.
  • Performs other duties as assigned.

Additional Pediatric Clinic Support: 

  • Follow-up with patients per pediatrician request after appointments or procedures to assess progress or address concerns. 
  • Lab and Imaging Follow-up: Communication as requested by the pediatrician.
  • Reviews and processes portal messages for pediatric offices.
  • Patient assessment 
  • Scheduling Appointments 
  • Medication refill requests 
  • Providing education for chronic and acute illnesses  

Performance Requirements: 

  • Ability to read, write, and understand the English language. 
  • Communicate effectively, interacting in a professional manner with a wide range.

  • Ability to operate electronic medical records, MS Word-Excel, and use of other

electronic communication such as email.

  • Ability to work independently with indirect supervision and autonomy.
  • Able to sit/stand for extended periods of time.
  • Able to bend and lift to fifty pounds.
    • Must have a workspace, if remote, that is compliant with patient privacy regulations and HIPAA laws.
