The Population Health Coordinator is responsible for supporting the coordination, monitoring, and optimization of population health initiatives across the medical group. This role involves tracking performance metrics, analyzing data from electronic health records (EHRs), identifying trends and areas for improvement, conducting patient outreach, and assisting in the implementation of Patient-Centered Medical Home (PCMH) principles. The coordinator works closely with healthcare providers and clinic teams to enhance care coordination, improve patient outcomes, and ensure compliance with quality standards and regulatory guidelines. This position will work from our 6th and Washington clinic in Spokane, WA.

What
you’ll do:
  • Conduct outreach to patients to schedule preventive screenings.
  • Collaborate with healthcare providers and clinic teams to ensure timely, effective communication and care coordination.
  • Provide comprehensive patient support by helping navigate healthcare services, follow-up care, and referrals.
  • Monitor and support patient engagement strategies to enhance participation in population health initiatives.
  • Support population health initiatives across multiple clinics.
  • Track and analyze population health performance metrics, identifying trends and opportunities for improvement.
  • Monitor and review data analytics from electronic health records (EHRs) to assess patient outcomes and adherence to quality metrics.
  • Assist in preparing reports and dashboards to support leadership in data-driven decision-making.
  • Participate in audits and assessments to ensure compliance with regulatory standards and best practices.
  • Assist in the coordination, implementation, and maintenance of Patient-Centered Medical Home (PCMH) principles across all clinic teams.
  • Support initiatives to improve care quality, patient outcomes, and health equity.
  • Monitor adherence to quality metrics, regulatory standards, and industry best practices to ensure compliance.
  • Participate in quality improvement initiatives, assisting in workflow enhancements and policy development.

What
you’ll bring:
  • Certified Medical Assistant license in Washington State.
  • Associate's degree in public health, Healthcare Administration, Nursing, or a related field or equivalent work experience.
  • One year of experience working in population health, quality improvement, data analytics, or care coordination within a healthcare setting.
  • Familiarity with electronic health records (EHRs), healthcare data analytics, and reporting tools.
  • Strong analytical and problem-solving skills to assess data and identify improvement opportunities.
  • Excellent communication and interpersonal skills to engage with patients, providers, and clinic teams.
  • Ability to manage multiple tasks, meet deadlines, and work collaboratively in a team-based environment.
  • Knowledge of quality metrics, regulatory requirements (e.g., CMS, HEDIS, NCQA), and healthcare compliance standards.
  • Proficiency in Microsoft Office Suite (Excel, Word, PowerPoint) and data visualization tools.
  • Bachelor's degree in public health, Healthcare Administration, Nursing, or a related field or equivalent work experience. (Preferred)
  • Experience in a primary care setting. (Preferred)

Working Environment
This position operates in a clinic environment, providing both in person and telehealth care. Collaboration with various healthcare providers is routine, and participation in wellness programs may also be required.

Physical Requirements
Ability to work in a clinical environment, interact with patients for prolonged periods, and perform duties requiring a high level of concentration and emotional resilience.
Vaccine Requirement:
Kinwell currently requires all teammates to provide proof of or complete a written attestation of a religious or medical exemption for influenza, COVID-19, and Hepatitis B vaccines. Healthcare providers may also be subject to CDC recommended vaccines.

The pay for this role will vary based on a range of factors including, but not limited to, a candidate’s geographic location, market conditions, and specific skills and experience.
National Salary Range:
$51,000.00 - $76,500.00
