The Population Health Data Coordinator is responsible for managing, analyzing, and interpreting healthcare data to enhance patient outcomes and healthcare delivery. This role involves working collaboratively with clinical and administrative teams to identify trends, optimize care strategies, and ensure the efficient coordination of patient care. The Data Coordinator will work closely with clinicians, healthcare professionals, and decision-makers to develop insights that drive strategies for improving population health and care management. Additionally, this role will support patient outreach and care coordination, including appointment scheduling and facilitating referrals for Population Health patients. The Population Health Data Coordinator will work on-site at one from our North Country Homes clinic in Spokane, WA.

What
You’ll Do:
  • Collect and maintain large healthcare datasets from multiple sources, including electronic health records (EHRs), insurance claims, and public health reports.
  • Ensure data integrity, accuracy, and compliance with privacy regulations (e.g., HIPAA).
  • Communicate actionable insights to clinical and administrative teams to drive healthcare improvements.
  • Track progress toward population health goals, such as reducing hospital readmissions or improving chronic disease management outcomes.
  • Provide patient outreach, such as appointment scheduling, follow-up care, and outreach to patients attributed to Kinwell but not yet established with a provider.
  • Facilitate timely referrals to specialists by prioritizing appointments based on urgency and following up with specialty clinics to understand their booking timelines
  • Work with IT professionals, data scientists, and healthcare providers to ensure accurate data flow and integration into healthcare systems.
  • Develop dashboards, visualizations, and reports to present data findings to non-technical stakeholders.
  • Assist in designing and implementing population health strategies that improve patient engagement and health outcomes.
  • Maintain accurate patient records, manage population health data, and upload pathology results and diagnostic testing outcomes into charts to track patient results effectively.
  • Assist nurses and care managers by tracking care plans, performing data entry, scheduling appointments, and ensuring timely follow-ups for high-risk patients.

What
You’ll Bring:
  • Associate’s degree in healthcare administration, public health, or a related field; or equivalent work experience.
  • Experience in data reporting and analysis within a healthcare environment.
  • Strong analytical skills and attention to detail.
  • Proficiency in Excel.
  • Excellent communication and presentation skills for diverse audiences.
  • Ability to manage multiple tasks, prioritize effectively, and meet deadlines.
  • Collaborative mindset and ability to work across interdisciplinary teams.
  • Demonstrated knowledge of healthcare regulations, healthcare data systems, EHRs, and population health tools.
  • Bachelor’s degree in healthcare administration, public health, data analytics, or a related field. (Preferred)

Working Environment
  • Work is primarily performed in an office setting within a healthcare organization, which may include proximity to patient care areas.
  • The work environment is generally quiet, but may involve some interruptions, high-paced demands and interactions with various departments.
  • This role requires the ability to navigate within clinical or administrative areas of a healthcare organization.

  • This is primarily a sedentary role with prolonged periods of sitting at a desk and working on a computer.
  • Ability to life or carry items weighing up to 25 pounds; occasionally may need to bend, stoop, or reach to retrieve items.
  • This role requires the ability to keyboard and computer for extended periods of time and to communicate clearly and understandably in person, and over the telephone.
  • Manual dexterity for data entry and use of office equipment.

The pay for this role will vary based on a range of factors including, but not limited to, a candidate’s geographic location, market conditions, and specific skills and experience.
National Salary Range:
$47,100.00 - $70,700.00
