LPN-Assisted Living-BAYLOR
7:00pm-7:00am Every Weekend (Saturday night/Sunday night)
New pay rates in effect with shift differential and $5000 sign on bonus
PURPOSE OF POSITION:
¨ Practice practical nursing according to Alabama Nurse Practice Act Maintains a management level position, responsible for distribution of nursing duties on the Assisted Living/SCALF unit and for providing comprehensive nursing care to residents based on the physician's medical care plan and the physical and psychological needs of the patient
MAJOR RESPONSIBILITIES:
¨ Provides, observes, and supervises nursing care for residents
¨ Cares for chronically, acutely, seriously ill residents
¨ Observes, records, and reports objective and subjective symptoms; Remains alert to any change in resident’s condition that may require revision of medical treatment and notifies the Director and physician
¨ Initiates (when appropriate) care plan and modifies and updates the plan, based on patient’s condition, doctor's orders, and nursing judgment; uses plan to provide care
¨ Accurately interprets and initiates doctor’s orders; Receives telephone orders from physician and carries out the orders
¨ Communicates observations of patient condition verbally and in writing in nurse’s notes clearly, concisely, and accurately using correct terminology
¨ Keeps family informed regarding patient’s condition and/or needs
¨ Provides prescribed treatments as ordered Ensures that vital signs are taken on residents seeing doctors the following am
¨ Obtains specimens from residents, records, and reports results of diagnostic tests and treatment procedures
¨ Ensures that monthly vital signs and weights are taken and recorded on each patient
¨ Inspects resident care area to ensure a comfortable, orderly, clean and safe environment for residents
¨ Must know and have working knowledge of Kirkwood policies, procedures and medical and nursing forms
¨ Assists in miscellaneous departmental duties/functions as needed
¨ All other duties as assigned
EDUCATION, EXPERIENCE, AND SKILLS REQUIRED:
¨ High school diploma/GED
¨ Must have an active LPN License in the state of Alabama
¨ Five plus years of applicable nursing home experience preferred
¨ Must be able to read and interpret medical records, patient's chart, diagnostic reports, and make entries as required
¨ Must know admission, transfer and discharge procedures and be able to carry out when necessary
¨ PC skills using Microsoft Office
¨ Excellent written and verbal communication skills
¨ Requires experience and training in maintaining optimum compliance with the state agency that regulates and surveys Assisted Living Facilities
¨ Participates in continuing education in management and special geriatric care
¨ Must be able to work with team members and work with minimal supervision
PHYSICAL REQUIREMENTS:
¨ Position may require the ability to lift up to 50 pounds
