What are the responsibilities and job description for the Case Manager position at Knoxville Area Project Access?
Nonprofit is looking to hire an ambitious, team player to join our Case Management team. This position is a crucial piece of our program and requires great organization and communication skills. Recent college grads encouraged to apply.
Responsibilities
· Assisting patients in the navigation of the healthcare system while enrolled in KAPA
· Coordinating all medical referrals for patients
· Working with provider offices in coordination of referrals
· Making patient reminder and follow up calls for appointments
· Providing education and tools for patients to be better healthcare citizens including:
1. Proper Emergency Department usage
2. Facilitating information from providers regarding patients’ medical questions
· Coordination of any non-medical patient needs including: transportation assistance, food assistance, reduced cost prescriptions, etc.
· Thoroughly documenting all patient information and interactions into KAPA’s patient computer program
· Participation in specialty projects as necessary
· Assistance in annual fundraiser with event attendance
· All other duties assigned by Director of Patient Services
Requirements:
· Passion for helping others
· Bachelor’s degree or equivalent
· Proficient computer skills including knowledge of Microsoft Suite
· Excellent time management and organizational skills
· Prior healthcare or insurance knowledge a plus
· Prior Case Management experience a plus
· Bilingual in Spanish a plus
This position is full-time (days, M-F), in office, with great benefits including medical, dental, vision, 401k, PTO, and room for advancement.
Salary : $35,000 - $40,000
