POSITION OVERVIEW:
Under the supervision of the Clinical Supervisor, the Promotora is assigned to the Mobile Mental Health program and will be deployed with a Staff Therapist to communities throughout Stanislaus and Merced Counties with the goal of increasing mental healthcare access. The Promotora will perform outreach and engagement, case management services, and linkage to other resources as needed. The Promotora will use cultural humility and strength-based approaches to ensure a compassionate and supportive environment. The Promotora will collaborate with government agencies, community-based organizations, school districts, and other healthcare providers. The Promotora will meet deliverable expectations and requirements of the program. This role will uphold and champion the agencys mission, vision, and values and contribute to a collaborative and inclusive work culture.
Essential Job Functions and Responsibilities:
Community Support and Advocacy:
- Build trust and rapport with community members.
- Serve as a role model and coach to inspire, engage and advocate for community members.
- Provide psychoeducation regarding mental health, substance, child development, and parenting skills.
- Provide culturally responsive support, including aiding individuals to understand and move through unfamiliar and complicated systems.
- Support community members with completing applications for social services programs to access resources to meet their basic needs.
- Utilize motivational interviewing and other evidence based interventions to assist community members.
- Collaborate with the team and other providers serving community members to ensure that all needs are being met.
- Network with schools, medical clinics, social services and other community partners/staff to support objectives of the assigned program.
- Use mobile van to bring services directly to communities.
- Conduct community outreach and engagement efforts.
- Assist with referrals and screenings for services.
- Provide case management services.
- Assist with navigating and accessing services.
- Facilitate educational and support groups.
- Assist with workshops and wellness events.
- Partner with trusted agencies to enhance service delivery.
Program Administrative Support:
- Participate in team meetings, training sessions, and other meetings as required.
- Provide appropriate documentation of services provided to clients.
- Support with data entry and provide help with reporting.
Documentation and Compliance
- Complies with all agencys policies and procedures and maintains strict HIPPA guidelines.
- Submits records of service information and tracking consistent with program policies.
- Maintains records, prepares reports and performs other administrative duties as required.
- Follows appropriate charting and documentation requirements and agency regulations in a timely manner.
- Provides input and feedback on potential related projects/programs, as appropriate.
- Supports agency-wide Quality Improvement efforts.
Other
- Performs other related duties as assigned.
- Attend trainings and participate in events relevant to key responsibilities
- Track and regularly report on key metrics for responsible function areas
- Collaborates on special projects related to agency objectives as appropriate
QUALIFICATIONS & EXPERIENCE:
- High School Diploma or equivalent preferred.
- Lived Experience with mental health challenges, homelessness, and/or substance use recovery preferred (self or family member).
- Ability to work independently, demonstrating excellent critical thinking skills and professional judgement.
- Ability to meet attendance requirements of the position and be timely in meeting all requirements for work performance. Proficiency using Google Suite and Microsoft Office.
- Bilingual in Spanish/English strongly preferred.
- Ability to work in an independent environment.
- Ability to learn web-based platforms (e.g., Salesforce) with appropriate training provided.
- Non-Profit and Community Based Organization experience highly desirable.
- Demonstrates and models excellent customer service orientation, especially in the ability to interact with people at all levels within and outside the agency.
- Thinks independently and can make informed decisions when needed.
- Exercises extreme discretion with the ability to hold, balance and transmit public and confidential information needed by various parties.
- Proficient using MS Office products, particularly Word, Excel, Outlook and PowerPoint.
- Acquires or maintains proficiency in using Google Drive products, including Docs, Sheets, and Forms.
