Description

POSITION OVERVIEW:

Under the supervision of the Clinical Supervisor, the Promotor(a) will be responsible for providing services to clients in their assigned programs at La Familia Central Valley. The Promotor(a) supports assigned Medi-Cal members (adults/children) by providing comprehensive care coordination and health promotion/health literacy development services to help participants navigate and overcome barriers to regular and reliably access and proactively engage in their care, inclusive of their primary care, mental health, substance use treatment, and the social determinants of health (which include basic needs support, like housing and food access). 

The promotor(a) will also help participants create and implement habits and strategies to better manage their health to improve selective health metrics and reduce unexpected and/or emergency visits to institutions which can be traumatic; and to grow their ability to regularly and reliably navigate systems and coordinate care independently or via the support of a family member. The Promotor(a) will provide high-touch, culturally responsive and culturally humble services with an emphasis on in-person service delivery. In addition to the direct service responsibilities, the Promotor(a) will support the implementation of the program by providing support to program leadership. This role will uphold and champion the agency’s mission, vision, and values and contribute to a collaborative and inclusive work culture.

Essential Job Functions and Responsibilities: 

Outreach:

  • Leverage existing and form new partnerships to represent cross-sections of participants throughout the county. 
  • Develop relevant, culturally appropriate, semi-structured, and qualitative questions with focus on participant experience accessing or receiving county services. 
  • Assist with planning and administering focus groups and completing surveys.
  • Thoroughly record and assist with analyzing and reporting findings.
  • Meet all project requirements and goals.

Care Coordination and Referral toward Health Action Plan Goals:

  • Develop and maintain rapport with participants (a broad term, which includes Medi-Cal members and their primary caregiver/legal guardian) and maintain rapport and trust through consistent, regular meetings, including proactively communicating and soliciting updates from participants and consistent use of Motivational Interviewing techniques.
  • Provide timely, thoughtful, evidenced-based, trauma-informed, culturally responsive responses to participants and all partners.
  • Conduct initial assessments and work with the clinical consultant team and supervisor to finalize each assessment.
  • Establish relationships with current providers for each participant and exchange data sharing consents to solicit information to inform Health Action Plan and on-going service delivery.
  • Create and manage progress toward collaborative Health Action Plan with each individual/family based on their unique goals and barriers.
  • Create and implement tools to ensure regular communication between the participant and all providers, increasing preparation and attendance for all appointments.
  • Help the participant coordinate paperwork and relay information between providers. 
  • Advocate on the participant’s behalf with new and existing providers. 
  • Provide social service and benefit enrollment referrals (including housing, Social Security Disability benefits, CalFresh, etc.) and follow up to ensure referral utilization, with an emphasis on reducing stressors and barriers to regular, reliable care.
  • Support participants through transitions of care, including helping participants to access timely information, determine options, set criteria for choices, and provide information to all providers.
  • Support participant access to new providers where there is an unmet need.
  • Support participant access to Central California Alliance for Health, Health Plan of San Joaquin, and Health Net for Health benefits, including transportation support. 
  • Provide crisis intervention as needed, including de-escalating and stabilizing clients in crisis through brief intervention and escalating support for the client to clinical consultants and other available interventions. 

Health Literacy Skills Development:

  • Assist in counseling participants and members of their broader support network in gaining additional health literacy skills including system navigation, self-advocacy, and accessing diagnosis-specific credible health information. 
  • Create and implement tools to support habit development and behavior change toward Health Action Plan goals.
  • Attend appointments with participants, as appropriate, and reinforce information gained in appointments. 
  • Role model techniques related to the following: information gathering (asking questions, reviewing appointment summaries), care coordination, goal setting, and self-advocacy.
  • Provide feedback and guidance about behaviors that will jeopardize regular and reliable access to care and/or health metrics.

Responsibilities:

  • Collaborates with their supervisor and/or staff therapist to develop overall program design, implementation, evaluation, and strategic planning and program growth.
  • Supports the writing and submission of all necessary documents and reports to funder by deadlines.
  • Provides mentorship and training to other team members, as appropriate.
  • Acts as a proxy for program leadership at funder meetings when program leadership is out of the office.

Other 

  • Provide effective outreach and timely response to engage all referrals made by funder and social service partners, including attending health fairs, making community presentations, etc.
  • Maintain networking and referral relationships with relevant social service partners and other service providers.
  • Work closely with clinical consultants to determine next steps if the participant's health status and/or cognitive/emotional ability to participate in the program appears to decline.
  • Attends monthly Care Coordination meetings. 
  • Maintains all records in accordance with funder and program requirements.
  • Attend trainings and participate in events relevant to key responsibilities
  • Track and regularly report on key metrics for responsible function areas
  • Leads and collaborate on special projects related to agency objectives as appropriate

Qualifications


QUALIFICATIONS & EXPERIENCE:

  • High School Diploma or equivalent preferred.
  • Lived Experience with mental health challenges, homelessness, and/or substance use recovery preferred (self or family member).
  • Ability to meet attendance requirements of the position and be timely in meeting all requirements for work performance. 
  • Bilingual in Spanish/English strongly preferred.
  • Ability to work in an independent environment.
  • Ability to learn web-based platforms (e.g., Salesforce) with appropriate training provided.
  • Non-Profit and Community Based Organization experience highly desirable.
  • Demonstrates and models excellent "customer service" orientation, especially in the ability to interact with people at all levels within and outside the agency.
  • Thinks independently and can make informed decisions when needed.
  • Exercises extreme discretion with the ability to hold, balance and transmit public and confidential information needed by various parties.
  • Proficient using MS Office products, particularly Word, Excel, Outlook and PowerPoint.
  • Acquires or maintains proficiency in using Google Drive products, including Docs, Sheets, and Forms.
  • Valid CA Driver's license
