Activity Assistant – Job Description
Reports to: Activity Director
Qualifications:
High School Diploma.
Graduate of state-approved Activity course preferred.
At least one-year experience in a skilled nursing care facility.
Dementia-specific training preferred.
Job Objective:
The primary purpose of this job is to provide direct activity therapy for residents as described in the
interdisciplinary plan of care in accordance with current applicable federal, state, county, and facility
regulations, policies and procedures, and as may be directed by the Activity Director.
Essential Functions:
Assist the Activity Director in completing the resident admission interview and assessment.
Assist in the completion of the assigned sections of the MDS (Minimum Data Set).
Write comprehensive Progress Note as directed.
Record daily activity attendance.
Understand procedures for all codes and responds appropriately to them.
Order/purchase supplies as needed and directed.
Assist with Resident council as directed, prepares minutes.
Organize and lead individual, small and large group activity programs as directed.
Set up and operate audiovisual equipment for activities.
Utilize computer with Microsoft word.
Digital photography experience and the ability to interface with computer for resident identification and
for publicity purposes.
Understand and follow the departments scheduling of work, meals and breaks. Work schedule will
fluctuate according to resident needs, including evenings and weekends.
Offer drinking water and other nourishments to residents as part of an organized activity.
Provide individual activities for residents in isolation and observe facility policies for aseptic techniques
and universal precautions.
Shop for assigned residents for personal needs as directed.
Transport residents to and from various activities as directed.
Report any changes observed in condition or behavior of residents.
Assist in maintaining a safe and clean environment by cleaning Activity Department and equipment.
Establish and maintain interpersonal relationships with residents, staff, volunteers and visitors while
assuring confidentiality of resident information.
Attend in-service education programs, as assigned, to learn new skills. From these educational
opportunities, makes suggestions for new ideas and programs as requested by Activity Director.
Practice careful, efficient and non-wasteful use of supplies.

workers, supervisory staff and visitors.
