Summary
We are seeking a Case Manager to join our team at Lafayette House. In this role, you will play a vital part in supporting clients through their healing journey by providing comprehensive case management services. You will work closely with individuals and families to assess their needs, develop personalized plans, and connect them with essential resources.
Responsibilities
- Provide immediate support and resources during and after a crisis.
- Help clients develop and implement safety plans.
- Connect clients with appropriate resources, including counseling, support groups, and other services.
- Advocate for victims’ rights and needs across multiple systems or areas of support, including but not limited to, criminal justice and legal, medical, housing, and within a victim’s family support system.
- Assist clients with preparing and submitting requests for temporary orders of protection or completing written victim impact statements.
- Assist clients with filing state compensation claims.
- Monitor client progress and adjust service plans as necessary to ensure effective outcomes.
- Assure collaboration and partnership with other agencies and service providers to ensure coordinated victim services.
- Conduct outreach to identify and engage with potential clients in a setting more appropriate to individual needs when applicable.
- Maintain accurate documentation of client interactions and case progress in compliance with confidentiality standards.
Requirements
- High school diploma and equivalent.
- Minimum of two years’ experience in a healthcare or social services setting, preferably in a behavioral health setting.
- Demonstrated proficiency regarding the use of an electronic database system for purposes of documentation and use of Microsoft Office applications.
- Experience working in a trauma-informed environment serving vulnerable populations.
- Excellent communication skills with the ability to manage conflicts effectively.
- Demonstrated ability to handle crisis situations calmly and professionally.
A bachelor's degree in social work or a related field is preferred.
