SUMMARY

Supervises staff employed in a community based home for persons with developmental disabilities and/or developmental disabilities. Position will develop, implement, and evaluate a residential services program and provide QIDP (Qualified Intellectual Disability Professional) services for residents. Performs related work as required. This person will work when necessary, which includes possible nights, weekends and holidays.

ESSENTIAL FUNCTIONS

Serve as the Home Manager and a role model for subordinate staff at the residence. Ensure the overall health and safety of recipients and strive to have recipients understand and exercise their rights to the fullest extent possible. Providing training to recipients in the development of independent living skills, including, but not limited to, instruction in social skills, meal preparation, money management, sexuality, and hygiene. Transport recipients as required. Assure completion of fiscal and other documentation responsibilities. Provide supervision and training to direct support professionals in all areas of program responsibility.

JOB DUTIES

SUPERVISORY RESPONSIBILITES

The Community Home Manager will work along with the Director of Residential Services and the QDDP:

· Provides supervision of all staff (CMAs and Direct Support Professional), including evaluations, discipline, and in-service training and carry out those responsibilities in accordance with Lafourche Arc policies and applicable laws for one community home.

· Participates in the interviewing, hiring, orientation, and training of all direct support professionals in accordance with agency policies and procedures.

· Maintains staffing at levels appropriate to the needs of the recipients, and in compliance with contracted levels. This may involve covering or finding coverage for shifts.

· Demonstrates leadership by effectively using a team process and by serving as a role model to staff.

· Assures that required documentation is done in a complete, legible and timely fashion. This includes the ISP and monthly progress reports, keeping information in the daily log, time cards, training records, mileage logs, absentee report, incident reports, and staff evaluations among others.

· Trains new CMAs and Direct Support Professionals in all aspects of their position including staff/consumer relations, agency discipline, guiding principles of work ethics and agency practices.

· Assists with the scheduling, preparing for and conducting staff meetings. This includes preparing an agenda, soliciting input from staff and completing meeting minutes.

· Supports staff by providing training including but not limited to the use of the Lafourche Arc internal support systems as well as existing community systems such as transportation, medical services, stores, banks, and community recreation and leisure programs. In addition, train staff in the use and role of assessment in developing and providing programming for recipients.

ADMINISTRATIVE RESPONSBILITIES

· Work effectively with QDDP and maintain close communication as required with other administrative staff.

· Know and understand, and follow LDH Licensing Standards, Lafourche Arc Personnel Policies, and the Lafourche Arc Residential Policies and Procedures and assure likewise for all staff. Assure maintenance of all records in accordance with these policies.

· Responsible for the day to day operation of the Community Home according to established policies and procedures pertaining to resident call.

· Attend Supervisor’s meetings and in-services as required.

· Ensure total compliance with OCDD, LDH, and State/Federal Licensing regulations, which includes employee training, monthly reports, evaluations, current plans, data sheets, medical information, financial records, etc…, in a timely manner.

· Assure that staff time cards are accurate. This requires matching schedules or the log with time cards, calculating time worked and turning in time sheets within required time frames.

· Ensure a secure, well-maintained home by setting a high standard for routine home and vehicle maintenance, including a focus on preventative maintenance and completing repairs in a timely manner.

· Assure the safe keeping of all recipients’ money by being responsible for keeping records and receipts as required, and handling banking.

· Being responsible for petty cash including staying within budget, keeping accurate records of expenses, and maintaining receipts as needed for living expenses.

· Review all incident reports, check for completeness and accuracy and submit according to required time frames and guidelines.

· Perform any other duties as assigned to meet the agency and the community home’s needs.

INDIVIDUAL RECIPIENT RESPONSBILITIES

The Community Home Manager will support the recipients by:

· Making a commitment to learning what individuals want, assuring that they have sufficient experience to be able to make choices about what they want, and helping them to realize goals and achieve outcomes

· Assisting the QDDP to plan and implement the individual habilitation plan (ISP) meeting, including setting up the meeting, inviting IDT members, writing the document, monitoring the implementation of the plan, and reconvening the group if any modifications are required.

· Completing assessments on at least an annual basis in order to identify needs and seek out the proper resources to address them.

· Teaching staff to recognize each individual recipient’s strengths and adaptive limitations to assure each individual is receiving his/her required level of support across all environments.

· Assisting the supervising staff and recipients in maintaining a routine of communal living by ensuring that all staff serves as a supportive companion, advocate and teacher to each recipient by providing effective on-site training.

· Teaching daily living skills as detailed in the recipient’s ISP and insures that the other staff is doing so also.

· Protecting individual’s rights, teaching them to exercise their rights, and supporting them to live up to their responsibilities.

· Observing each recipient’s right to confidentiality.

· Maintaining a professional, liaison and communication with recipient families, guardians, employers, day program staff, other agency staff, LDH staff and professionals in the community such as physicians, counselors, etc.

· Participating in and planning recreational/leisure activities with the recipients and the staff.

· Being responsible for medications including administration, record keeping, proper storage, proper quantities, review and follow-up of errors, etc. Additionally, scheduling regular medical and dental appointments to assure the recipient timely effective medical treatment both as required and needed, thus ensuring each recipient a safe and healthful life.

· As assigned, being prepared for, being present at, and conducting meeting involving the recipients. (Admissions, ISP, House Meetings).

· Assisting with maintaining organized and up-to-date resident files.

OTHER SKILLS AND ABILITIES REQUIRED

· Maintains a professional liaison and communication with recipient families, guardians, employers, day program staff, other agency staff, Louisiana Department of Health (LDH) staff, and professionals in the community.

· Demonstrates leadership by effectively using a team process and by serving as a role model to staff.

· Demonstrates accountability and responsibility for own work.

· Acquires, maintains, and demonstrates current knowledge and competency.

· Demonstrates a commitment to service excellence and quality improvement.

· Ability to provide transportation as needed.

· Ability to work a schedule based on operation needs (On-call, weekends, holidays).

· Ensures that health and safety standards are sufficient in meeting compliance with all applicable rules.

SUPERVISORY RESPONSIBILITIES

Directly supervises Lead CMAs, CMAs, and Direct Support Professionals assigned to the Residential Services Division. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include interviewing and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE

High School Diploma/GED required with one-year experience working with individuals who have cognitive/physical disabilities in a residential facility. Must be a Certified Medication Attendant. Knowledge of OCDD, LDH, and State/Federal Licensing regulations is necessary.

LANGUAGE SKILLS

Ability to read, analyzes, and interprets general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, recipients, customers, and the general public.

MATHEMATICAL SKILLS

Ability to add, subtracts, multiply, and divides in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage and to draw and interpret bar graphs.

REASONING ABILITY

Ability to define problems, collects data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. Ability to exercise independent judgment. Ability to assess, project and plan for recipient needs. Ability to understand principles of behavior management. Ability to interpret state/federal/agency regulations. Ability to document concisely, accurately, and timely.

Work is active and is performed within the recipient’s home as well as in the community. The behavior of consumers may change dramatically due to environmental or medical conditions among others, which may, at times, create hazardous situations. Exposure to infectious diseases, blood and bodily fluids occasionally. Noise level is moderate. Exposure to inside and outside environmental conditions. You may provide individual support offered in a dignified fashion in the areas of toileting, showering, menstruation, and any other areas of hygiene, ranging from intermittent to pervasive levels.
