What are the responsibilities and job description for the Public Health Nurse II position at Lake County General Health District?
Position Summary:
This position will serve as a member of the team to address departmental priorities and develop and strengthen neighborhood connections with the most vulnerable populations. Provide professional public health nursing services to the community and provide nursing surge capacity in an emergency. The RN Public Health Nurse position is funded through the Board of Health and may be supported by other grants with Lake County General Health District.
This position may be at risk for limited occupational exposure to blood borne or communicable disease. Additional exposures may include but are not limited to: radiation, asbestos, mold, tobacco smoke, rabies, chemical substances, pesticides and unidentified odors.
Essential Duties:
Analyzes and applies data to the community’s present and projected health needs by proposing and planning specific programs;
Assists Director in determining the scope of programs, and in the development of long range plans for Health Promotion;
Plans and implements effective health education/promotion programs
o Develops a written annual work plan for the program or project;
o Establishes and monitors time-limited and measurable objectives and outcomes;
o Identifies target population, supportive methods and activities to be used, time lines and an evaluation process;
o Identifies the media which is most appropriate for dissemination of specific health information;
o Prepares and presents educational sessions or events as needed;
o Requests program/materials purchase through the Director;
o Evaluates and redesigns programs or projects based on outcomes;
o Prepares periodic reports on activities and achieved outcome;
Communicates and collaborates with community members and other agencies for programming;
Utilizes resources to avoid duplication of services/programs;
Participates in quality improvement activities;
Must have knowledge of State of Ohio nursing licensing requirements.
Other Duties & Responsibilities:
Maintains compliance with grant guidelines, if applicable;
Prepares and submits to supervisor a monthly and annual report of program activities and progress;
Attends various meetings that are scheduled by Unit, Division or Health District;
Maintains a system of record keeping for files, materials and purchases;
Attends grant meetings and maintains knowledge in the field;
Receives and screens community requests for information and assistance;
Identifies and implements public health laws, regulations and policies;
Actively participates in research activities in practice;
All health district employees have an emergency response role and may be expected to respond to threats to the public’s health in accordance with the Lake County General Health District Emergency Response Plan;
Performs other duties as assigned consistent with position.
