LPN Coordinator Home Care and Wellness is accountable to organize and manage the Home Care/Wellness Clinic to deliver professional Health Care services, wellness and Home Care Coordination of services. Responsible for oversight of the resident Wellness Clinic and works closely with Residential Living Residents and other departments to aid residents in maintaining optimal level of care. Coordinates and oversees the staffing of Home Care services.
A. Minimal Education:
1. Graduate of accredited school of nursing.
2. Registered Nurse or License Practical Nurse
B. Minimal Experience:
1. Minimum of three (3) years Nursing experience working with the elderly.
C. Other Qualifications:
1. Current Nursing License.
2. CPR certification.
3. Strong communication, leadership skills and interpersonal relationship skills.
4. Demonstrated clinical competency and provision of expert care to patients with either predictable or unpredictable.
