The lake Whatcom Center is seeking a dedicated and compassionate Case Manager, fluent in English and Spanish, to join our team.
As an Intensive Outpatient Case Manager, you will play a vital role in providing support and guidance to clients in need. Responsibilities:
- Must be 21 years of age or older.
- Conduct needs analysis with clients and develop individualized care plans
- Collaborate with healthcare professionals to coordinate and facilitate client care
- Provide education and support to clients
- Monitor client progress and adjust care plans as needed
- Assist with care coordination and ensure appropriate follow-up care
- Advocate for client's rights and ensure they receive necessary resources and services
- Maintain accurate and confidential client records in compliance with HIPAA regulations
- Utilize case management software to document client interactions and track progress
- Maintain a minimum productivity ratio of 40%
Experience:
- Associate's, Bachelor's, or Master's degree in social work or related field
- Previous experience in case management, preferably in a healthcare setting
- Knowledge of pediatrics, behavioral health, and addiction counseling
- Familiarity with utilization management principles
- Strong communication skills, both written and verbal
- Ability to work effectively as part of a multidisciplinary team
- Excellent organizational skills and attention to detail
