DETAILED SUMMARY:
SEAS (Single Entry Access to Services) Case Manager position is responsible for case management for children and youth in Whatcom County ages birth – 21 with special healthcare needs, prenatal parents and parents with a child birth-5, and parents in need of perinatal behavioral/mental health supports. The role of the SEAS Casemanager is to provide more support to families that goes beyond the role of the SEAS navigator.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
1. Receives and follows up on referrals from SEAS navigators where the family needs more support
beyond what SEAS navigation can provide. This can include families with literacy, cognitive, language, or other barriers
that makes it more difficult for families to access services, and they are not currently in a program that can assist.
2. Work alongside families to identify their needs, goals, and services they would like to access, including but not
limited to community and financial services, medical/health providers, developmental supports, postpartum
and parenting support resources, and basic needs.
3. Develops a service plan with the family that is family centered, outlines family’s needs and goals, activities to
meet goals, and a timeline to achieve goals and resource connection.
4. Once the service plan is developed, meet with families monthly (or more if needed) to help them access services and to discuss progress.
5. Assists families in completing applications, making calls to access services, or other tasks that might be needed to get the family connected.
6. Coordinate with other service providers as needed to support the family in connection to services.
7. Completes linkages and follow through back to referers once a referral has been received and followed.
8. Performs data input of family and child information using local data guidelines set by funders, using multiple databases and spreadsheets.
9. Writes case notes for each encounter with the family to track progress and resource connection.
10. Attend weekly staff meetings, department, and agency wide meetings as scheduled.
11. Provides backup navigation as needed.

BILINGUAL POSITIONS ONLY:
Specialize in providing services to clients and families who speak a language other than English as a primary language. May occasionally be asked to support with translating/interpreting a language other than English to assist potential or current clients.

OTHER RESPONSIBILITIES:

EDUCATION, EXPERIENCE, TRAINING REQUIREMENTS:
1. Associates degree or higher in Human Services, Social Work, Early Childhood Development, or another related field.
Previous work experience in a similar role could be used in lieu of a degree.
2. Proven ability to individualize programs to fit specific needs.
3. Knowledge of related resources or ability to access or learn resources.
4. High competency with documentation and record keeping.
5. Must be proficient in the use of computers and related information technologies for agency communications and
record keeping requirements
6. Proficient in the use of computers and related information technologies, including spreadsheet and/or database software.
7. Ability to work a flexible work schedule, including early evenings and weekends as needed.
8. Completed TB test and submit test results prior to hire date.
9. Criminal records check to ascertain that the applicant has not been convicted of child abuse or neglect offense
per Washington State Child Abuse Law (Ch. 486, Laws of 1987) or DSHS licensing requirements.
10. Current First Aid card and Infant/Child CPR training (or obtain within 30 days), and must maintain current certificates
for length of employment.
11. Valid Washington State Driver’s License or obtain by date of hire. Must submit abstract of driving record for previous 3
years prior to start date.
12. If hiring for a bilingual position. Must be able to speak, read, and write with proficiency and fluency in the required language.

2. An understanding of local social and health services systems.
3. A background in child development
4. A background in postpartum support resources
5. Ability to maintain confidentiality of sensitive information.
6. Demonstrated ability to relate in a supportive, non-judgmental manner to staff and families of
varying cultural, educational and socioeconomic backgrounds.
7. Excellent interpersonal skills with the ability to establish strong working relationships with families and providers.
8. Excellent observation and critical thinking skills.
9. Demonstrated independent work skills, leadership, flexibility, problem solving and decision-making abilities.
10. Excellent innovation skills; ability to assist in developing new methods, procedures, or systems.
11. Excellent time management skills and organizational skills.
12. Must be able to adapt interpersonal skills in response to individual styles, personalities and cultures.

WORKING CONDITIONS:
Work is performed in a fast–paced office environment with frequent interruptions and occasional crisis situations. Travel also includes attending local and statewide level meetings. Majority of work centers on client services and procedures that require a high degree of confidentiality, tactfulness and respect for clients from a diversity of economic and ethnic backgrounds. Most client communications will occur via in person, phone, email, and text. In person meetings may occur in the community and family’s homes. All job duties are completed while following current Opportunity Council COVID-19 safety guidelines.
