What are the responsibilities and job description for the HEART Intensive Outreach Case Manager position at Lake Whatcom Center?
Program Description:
The HEART (Holistic Engagement through Allied Recovery and Treatment) team is an intensive community outreach and crisis response program. HEART is a dynamic part of the Lake Whatcom Center’s wide array of services, created to increase access to comprehensive, high quality, community-based behavioral health services.
The HEART Team expands on existing whole-person, recovery-focused care services to increase crisis response and service accessibility while mitigating strain on traditional 9-1-1 services. The team is made up of Outreach Case Managers, a SUD specialist, MHP’s, an ARNP, and an RN. The HEART Team provides culturally competent intervention, assessment, case management, and advocacy to individuals in crisis.
Outreach Case Managers will work a set schedule of four 10-hour shifts (12pm-10pm). These hours and shifts are specialized to increase access to services outside of the normal 9am-5pm business hours.
Outreach Case Managers will be dispatched to respond or co-respond to behavioral health crises and emergency situations. Co-response efforts will occur in collaboration with community partners such as Mobile Crisis Outreach Team (MCOT), Whatcom Ground-level Response & Coordinated Engagement program (GRACE), law enforcement, and other first responders to provide wrap around services to individuals in need. The HEART Team will also provide follow-up and ongoing recovery support and case management services as necessary.
Minimum Qualifications:
* Preferably Bachelor’s Degree in behavioral sciences (Psychology, Sociology, Human Services, etc.) or related field, or Associate of Arts Degree with at least two (2) years’ experience in mental health or related field working with chronically mentally ill adults.
* Good personal relations skills; ability to work with difficult clients.
* Good organizational skills; Ability to work independently and manage multiple tasks.
* Ability to work cooperatively in a team approach.
* Ability to apply a Recovery based approach.
* Ability to exercise independent judgment and discretion.
* Good verbal, written, communication, and computer skills; visual and auditory skills required to communicate, ensure and deliver client care and provide legal documentation of care.
* Requires frequent standing, bending at the knees and waist, kneeling and reaching; Requires frequent walking to accomplish tasks; Requires ability to lift 30 pounds.
* Acceptable driving record and ability to be covered by LWC insurance carrier. Must have a valid Washington State Driver’s License or have a current valid driver’s license from another state and apply for a WA DL by hire date.
* Ability to apply for Department of Health Agency Affiliated Counselor License within 7 days of hire and obtain said license within 60 days. Ability to obtain CPR certification within 30 days of hire.
Employment contingent on successful background check and Washington State counseling profession credential issued by the Department of Health (e.g. Agency Affiliated Counselor and must apply for within 7 days of hire, preferably prior to hire).
