What are the responsibilities and job description for the Aging Advocate position at Lakes & Prairies Community Action Partnership?
JOB FUNCTION/SUMMARY:
Provide supportive services to the aging population. Build relationships with older adults to ensure stability by helping them navigate systems and resources within the community.
QUALIFICATIONS:
Minimum
- Associate’s degree in a Human Service-related field
- Valid driver's license and current vehicle insurance
Preferred
- Bachelor’s Degree in field relating to Human Services
- Possess an understanding and working knowledge of issues relating to the Senior Population
- Previous experience working with a Community Action Partnership Agency or other grant funded programs
- Lived experience or a history working with individuals with low incomes

ESSENTIAL FUNCTIONS:
Provide direct assistance to seniors to ensure independent living:
a) Provide direct assistance to seniors with identified needs
b) Assist persons to identify and obtain other needed resources to maintain independent living
c) Collaborate with County Public Health and Social Services to support seniors in crisis
d) Assist seniors in preparing their state property tax forms
e) Provide information, referral, and follow up services for seniors contacting the senior linkage line
f) Oversee the recruitment, enrollment, delivery, and distribution of the NAPS program
g) Execute outreach tables at community events
h) Complete and enter client data into data collection system
i) Coordinate the Group Workcamp home repair projects co-sponsored by CAPLP in conjunction with Group Mission Trips staff and local community partners.
Agency and Division Support
a) Represent Agency in communities we serve
b) Align and build on efforts to achieve a shared vision and common goals in serving people
c) Assist with Agency projects as requested
d) Attend and participate in agency, community and state meetings as requested
e) Participate in agency provided trainings and seek out additional training to enhance professional development
f) Assist with onboarding and training of Agency team members as requested
g) Compile and submit required documentation
h) Availability to work designated Agency hours for your Division and position
