The position requires adherence to IDD Provider principles and guidelines, regulations, and Lakes Regional Community Center's policies and procedures. The Collaborative Care Coordinator is responsible for developing, organizing, implementing, and monitoring an array of supported services for people with intellectual and developmental disabilities with mental health needs. Provides harmonious coordination of services with other LRCC departments and outside service providers for caseload assigned.
