What are the responsibilities and job description for the Case Manager position at Lakeshore Community Health Care?
The Behavioral Health Case Manager will visit, educate, and serve as a resource to providers and patients who need support for chronic disease management with a focus on mental health, substance abuse, social-emotional, and/or environmental issues. This person will work in a manner that supports both the patient and the health centers mission and performs in accordance with system-wide competencies and behaviors.
PRIMARY RESPONSIBILITIES
- Provides case management for identified LCHC Patients.
- Provides navigation of resources to all LCHC Patients
- Provides support and assistance in the delivery of care.
KEY COMPETENCIES & ABILITIES
- Relevant experience in case management.
- Ability to interact with physicians & other health care professionals in a professional manner.
- Working knowledge of physical health and behavioral health medications.
- Proficient in Microsoft Office
- Keeps all matters related to the organization confidential in accordance with HIPAA and the confidentiality policies and procedures.
EDUCATION & TRAINING REQUIREMENTS
- Masters Degree in Social Work Required
- Current CPR/Basic Life Support Certification upon hire or willing to obtain & renew as needed.
Job Type: Full-time
Pay: $59,400.00 - $69,800.00 per year
