The Opportunity:
The Case Manager provides community-based case management for individuals diagnosed with mental health and or substance use disorders. The Case Manager coordinates care individuals enrolled in Comprehensive Community Services, Community Support Program, Children’s Long-Term Support, or the Coordinated Services Team program by collaborating with a multi-disciplinary team. The population served includes individuals with a mental health and/or substance abuse diagnosis.
The pay range for this role is $26.00 - $30.90 based on education and experience.

What You’ll Do:
- Determine ongoing eligibility for Community Treatment programs via the approved state functional screen
- Facilitate the assessment and service planning process including the identification of consumer needs, goals, objectives, strengths, barriers, interventions, and their priority in the treatment planning process.
- Assess ongoing needs, including crisis situations, to determine treatment interventions and service provision.
- Ensure the service plan and service delivery for each client is integrated, coordinated, and monitored
- Provide services in adherence to the pillars of Community Treatment program philosophy which include being strength-based, recovery-oriented, person-centered, flexible, trauma informed and consistent with best practice.
- Complete documentation in compliance with Community Treatment standards, agency policy and applicable regulations.
- Adhere to scheduled work hours with regular and predictable attendance.
What You Bring:
- Case Manager I: Bachelors degree in human services field
- Case Manager II: Masters degree in human services field
- At least one year of case management experience.
- Ability to work in fast-paced environment.
- Person-centered service in all interactions.
- Valid driver’s license and use of a personal vehicle.
- Attention to detail and organized.
