Lakeshore Talent is in search of an RN Manager Home Care for a direct hire opportunity at two locations in Philadelphia, PA. This role offers a salary of $96,000 - $115,000 and an approved $7,500 sign-on bonus. The selected candidate will begin immediately.
Position Details:
- Schedule: Monday - Friday, 8 AM - 5 PM (On-Site)
- Software: Epic EMR
Key Responsibilities:
- Lead and manage a team of RNs, Schedulers, Medical Records personnel, and CNAs.
- Oversee both field-based clinical staff and office-based operational staff.
- Ensure efficient staffing, scheduling, and quality of care for home care participants.
- Conduct hiring, training, performance evaluations, and employee development.
- Monitor and manage departmental expenditures, including payroll and overtime.
- Serve as a primary participant in the Interdisciplinary Team (IDT) for collaborative care planning.
- Conduct audits and ensure compliance with organizational policies and regulatory standards.
- Perform initial and annual assessments of participants in their home environment.
- Develop and coordinate Plans of Care (POC) with scheduling teams and caregivers.
- Administer medications, monitor compliance, and provide health counseling.
- Ensure all medical records, reports, and participant data are accurately maintained.
Required Qualifications:
- Active RN license (as required by law).
- Bachelor of Science in Nursing (BSN).
- 2 years of RN healthcare experience, preferably with geriatrics.
- Leadership experience, including staff supervision and scheduling.
- Current First Aid and BLS certifications (American Heart Association or Red Cross accepted).
- Valid driver’s license and auto insurance.
Preferred Experience:
- Home Health, Home Care, Hospice, SNF, or LTC experience.
- Ability to work in a complex matrix leadership environment.
- Strong critical thinking, emotional intelligence, and collaboration skills.
