Overview:
The Health Information Specialist is responsible for appropriately and accurately indexing incoming records from our fax portal into the appropriate chart, as well as verify and process requests for records for patient care, quality review, and audits in a timely manner. The Medical Records Specialist will also be responsible for releasing information to patients, healthcare facilities and other entities while following all applicable local and national laws and regulations including HIPAA
Essential Functions:
Individuals must be able to perform each essential function of the position satisfactorily. The responsibilities of the Medical Records Specialist have been outlined below.
Responsibilities:
- Ensuring patient medical records are organized, accurate, and complete.
- Ensuring that documentation of professional services provided at hospitals and surgical centers are available in the Lakeview Clinic EHR, as well as available to the coding/billing department utilizing Excel and Epic CareEverywhere.
- Assist in filing of documents for the Quality Improvement department relating to our quality initiatives.
- Accurately keying in results based on specific documentation and tasking providers to review patient results.
- Utilizing a web-based application to sort and process electronic faxes for Lakeview Clinic.
- Preparing invoices for records release to law firms, insurance agencies and other entities; gathering all pertinent patient information and communicating this information to the requester.
- Process payments for records requests.
- Answering patient and employee phone calls pertaining to medical record inquiries.
- Processing requests to release patient information for coordination of care and patient requests.

Education and Experience:
- High school degree required; Associated Degree in business or healthcare management preferred.
- Minimum of 1 year experience in Medical Records
- Minimum of 1 year experience in customer service
Work Environment:
- Environmentally controlled office within a multispecialty medical practice.
- Occasionally high pressure or emergent situations in a fast-paced environment.
- May wear Personal Protective Equipment (PPE) such as gloves and mask
- Frequent interaction with a diverse population including team members, providers, patients, and other members of the public.
Physical Demands:
- Frequent sitting, standing, walking, reaching, bending, stooping, lifting up to 25lbs., carrying and speaking.
- Lifting, carrying, pushing and pulling up to 50 pounds, with assistance if needed.
- Frequent use of computer, keyboard, phone, copy and fax machines.
