Care Team Leader

**Responsibilities**

The RN will be responsible for:

• Developing and implementing individualized plans of care based on resident assessments;
• Selecting and instituting appropriate nursing interventions to stabilize resident conditions and prevent complications;
• Contributing to resident assessments and developing plans of care;
• Providing safe and accurate medication-related interventions to residents;
• Supervising the care provided by LPNs, CNAs/STNAs, and other team members;
