What are the responsibilities and job description for the Resident Care Coordinator position at Lakeview Terrace Assisted Living & Memory Care?

The Resident Care Coordinator is responsible for resident service plan and Health Services staff schedules, and other tasks as needed for resident well-being.
1. Develop and maintain resident service plan review and schedule according to service plan policy and state regulation for timely reviews with the resident and/or responsible party.
2. Discusses with Health Services staff any changes needing to be reflected in resident service plans such as: changes in physical, emotional, mental, or behavioral condition, unusual symptoms, accidents, injuries, or unusual occurrences and reports such instances to a licensed nurse, and/or the administrator.
3.Demonstrates knowledge of infection control and universal precautions.
4. Observes any changes in resident physical or mental status and communicates changes appropriately.
6. Displays mature behavior and attitude in speech and action; demonstrates consistently pleasant demeanor, tone of voice, supports the mission statement and approach to care used by the community.
7. Maintains an acceptable attendance record; notifies supervisor if late or unable to work in accordance with personnel polices; follows the assigned work schedule.
