Lakewood Villa is seeking a skilled and detail-oriented RN to join our leadership team as MDS Coordinator. In this essential role, you’ll work closely with our Director of Nursing to oversee care planning, lead the MDS assessment process, and support overall clinical excellence within the facility. This position is ideal for a nurse who is both data-driven and compassionate — someone who can balance regulatory expertise with a strong commitment to resident-centered care.

What You will Do:
- Partner with the Director of Nursing to support clinical operations and staff leadership
- Oversee timely and accurate MDS assessments, care planning, and documentation
- Coordinate with the interdisciplinary team to ensure compliance and care quality
- Participate in survey prep, QAPI efforts, and skilled services review
- Mentor nursing staff on documentation, care standards, and best practices
- Provide direct nursing support and leadership coverage as needed
What You will Need:
- Active RN (Registered Nurse) license in the state of Colorado
- 1–3 years of MDS experience preferred
- Minimum of 2 years of nursing leadership experience, ideally in post-acute or memory care settings
- Self-directed, self-motivated, and detail-oriented
- Strong ability to manage multiple projects and competing priorities
- Demonstrated ability to instruct, coach, and support staff
- Excellent communication and customer service skills
- Passion for resident-centered care and clinical excellence
