Role Overview
As a key contributor to the Lanai Community Health Center, the incumbent in this role plays a pivotal part in ensuring the effective operation of the organization. As a Referral Specialists, you will work closely with Lna'i Community Health Center (LCHC) patients, families, clinical staff, and external organizations to ensure quality patient care by assisting with patients' referrals, coordinating services and resources in regard to referrals, obtaining referrals notes, tracking hospitalizations/imaging reports/procedure/specialist notes, and monitoring follow-through of referrals for patients and the LCHC medical providers. LCHC is a patient care, teaching and research organization and provides workshops, education sessions and outreach - all employees should expect to be involved with these activities to the extent to which will depend upon their position in the organization.
Qualifications
- High school diploma or equivalent required
- Associates degree in social services or equivalent work experience (preferred)
- Demonstrated knowledge of community resources, health services, and social services available to Lanai and the State of Hawaii.
- Productive understanding regarding eligibility requirements, sliding scales, and social programs.
