- Monday - Friday, 8:30a - 5:30p
- This is a hybrid role with time spent at the facility and in the field visiting judicial referral partners.

- Up to $50,000 per year, based on experience

Summary:
The Court Communication Liaison is responsible for coordinating and managing the care and services for individuals within the court system. This role involves assessing clients' needs, developing care plans, connecting clients with appropriate resources, and ensuring follow-up to monitor progress. This role works closely with clients, their families, court contacts, and communicate partnerships to ensure that clients receive the necessary support to achieve their goals.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Client Assessment: Conduct thorough assessments of clients' physical, emotional, and social needs to develop individualized care plans or service plans.
- Care Planning: Develop, implement, and monitor personalized care plans based on clients' needs, preferences, and goals. Update plans as necessary to address changing circumstances.
- Resource Coordination: Connect clients with community resources, services, and support programs. Ensure access to health care, housing, employment services, and other necessary services.
- Case Monitoring and Follow-up: Monitor client progress, maintain regular follow-up, and adjust care plans as needed. Provide ongoing support and guidance to ensure clients’ needs are being met.
- Documentation and Record Keeping: Maintain accurate and up-to-date case files and documentation, including progress notes, care plans, and reports. Ensure compliance with organizational policies and confidentiality requirements.
- Advocacy: Advocate on behalf of clients to secure resources, services, or assistance needed for their well-being. Provide support for clients navigating complex systems, such as healthcare, housing, and legal services.
- Collaboration: Collaborate with other service providers, healthcare professionals, and community organizations to deliver coordinated care and support for clients.
- Crisis Intervention: Provide crisis intervention services, de-escalate situations when necessary, and connect clients with emergency services or counseling.
- Reporting: Prepare and submit reports related to client progress, case outcomes, and service utilization to supervisors, funders, or other stakeholders.
- Compliance: Ensure that all case management activities comply with applicable laws, regulations, and organizational policies and procedures.
