POSITION SUMMARY
- DIVISION: Senior & Disability Services / Case Management
- SUPERVISOR: Unit Manager
- UNION REPRESENTATION: SEIU
- POSITION PURPOSE: The Community Transition Coordinator provides long term care service intakes, case coordination, and transition services to eligible seniors and people with disabilities who are identified on a priority list, such as the Seniors by Name List as set forth by Lane County Human Services Division. Lane County Human Services Division will identify and prioritize individuals in need of the Community Transition Coordinator's dedicated support. This is a specialized position that will work closely with the shelter providers, CCOs, outreach, hospital social workers, Lane County HSD Coordinated Entry staff, Food for Lane County Dining Room, long term care facilities, natural supports, and other community-based resources to help individuals access long term care service plans and housing supports.
QUALIFICATIONS
Minimum
- High school diploma or GED and any combination of education and experience that provides the occupant with the skills, knowledge, and ability required to perform the job.
- Bachelor's degree in social work or related field
- Three years progressively responsible experience working within Senior and Disability Services or Oregon Department of Human Services working with seniors or people with disabilities.
- Basic computer skills and comprehension.
- Valid State of Oregon driver's license.
- Must pass a background check.
