Bilingual Wraparound Care Coordinator

The Latino Community Development Agency (LCDA) is actively looking for hard-working, creative, and dedicated individuals to join our agency and become integral parts of their team.

Definition

The Care Coordinator (CC) provides targeted support services to maintain a system of care that is child-centered around the needs of the whole family. The CC assesses the needs of the family through a non-clinical lens and provides services to the family in their homes and communities. A strong emphasis is placed on the strengths of the family as well as the child’s age and cultural heritage.

Position Requirements

  • (1) High School diploma or equivalent and 36 months of Behavioral Health experience or (2) 60 college credit hours in any field and 12 months of Behavioral Health experience or (3) Bachelor's or master's degree in any field with 6 months Behavioral Health experience.
  • Ability to effectively communicate verbally with Spanish/English speaking consumers
  • Computer literate
  • CC will be responsible for maneuvering various types of programs.

Essential Duties & Responsibilities

  • Performs duties consistent with training, experience, or assigned by the Project Director.
  • Meet with families in their homes, at hours convenient for families to provide wraparound services.
  • Schedule, coordinate, and facilitate Wraparound/family team meetings.
  • Complete initial wrap plans, strengths, culture discoveries, and intakes as needed (report). Update and modify wrap plans as needed.
  • Monitor and evaluate services provided to the families.
  • Possess skills necessary to provide immediate crisis intervention on a 24-hour basis.
  • Maintain close communication with the Project Director, FSP, and other clinical and administrative staff in order to assure the highest quality of services is delivered to the juvenile consumer and his/her family.

  • Ability to establish working relationships with clinical, administrative, and support staff in order to develop professionalism and coordination within the team.
  • Able to establish and maintain an effective working relationship with fellow employees, consumers, family members and the public.
  • Ability to see, hear, and speak clearly to interact with staff, consumers, family members, administration and the community, both in-person and on the telephone.
  • Ability to maintain strictest confidentiality guidelines for interactions regarding consumer and family information.
  • Ability to work well under pressure meeting deadlines with efficiency and consistency while maintaining a balance between good physical and mental health and the job requirements.
  • Such specialized training and/or certification in mental health services or a related field as may be required in order to provide adjunctive services, i.e. Juvenile Case Management Certification, SOC training, and updates, etc.
  • Personality and mature presentation of oneself demonstrating the ability to work with various systems while maintaining objectivity and cooperative attitude.
  • Current Oklahoma Operators Driver’s License.
  • Must be able to climb stairs and access non-handicapped equipped residences/facilities.

Position Requirements

  • Position requires education and experience sufficient for designation as a Certified Case Manager Level 2 and Behavioral Rehabilitation Specialist per the standards adopted by the Oklahoma Health Care Authority and the Department of Mental Health & Substance Abuse Services (Certifications may be acquired once hired).
  • Bachelor’s Degree in Psychology, Social Work, or related behavioral science field
  • Minimum of one (1) year experience in a mental health and substance abuse related field (educational experience may be allowable if recently graduated).
  • Ability to effectively communicate verbally with Spanish/English speaking consumers
  • Computer literate
  • Case Coordinator will be responsible for maneuvering various types of programs.
  • Traveling outside of work-site required
  • Valid driver's license with auto insurance required

Job Type: Full-time/Part-Time

Salary: $18.00 - $20.00 per hour
