What are the responsibilities and job description for the Service Coordinator position at Laurens County Health Department?
PURPOSE: Babies Can’t Wait is to provide early identification and screening of children with developmental delays and chronic health conditions birth to three years-of-age; to improve the developmental potential of infants and toddlers with developmental delays or chronic health conditions; to support family member and caregivers to enhance children’s learning and development through everyday learning opportunities.
JOB DESCRIPTION: Under general supervision, service coordinator will provide comprehensive case management for children served by Babies Can’t Wait early intervention program. Services will be provided through primarily home visits, some Webex teleinterventions possible. Duties will include completing initial screenings, intake interviews, coordinating the scheduling of evaluations, and assisting with the development of plans to provide services for the child/family. Service coordinator will continue to provide appropriate guidance and plan reviews, including transition from this program, throughout the child’s stay in Babies Can’t Wait. Service coordinator may provide special instruction for child/family toward meeting outcomes. Additionally, service coordinator will provide families with information around community resources. Service coordinator will learn and become proficient in the use of the Babies Can’t Wait database. All services provided within compliance with Health Insurance Portability and Accountability Act (HIPPA) and Family Education Rights to Privacy Act (FERPA) while working with a multidisciplinary team of professionals.
QUALIFICATIONS: Bachelor of Social Work or bachelor’s degree in any behavioral science from an accredited college or university.
PREFERRED QUALIFICATIONS: Candidate will have a related bachelor’s or master’s degree of one of the following: audiology, child development, child psychology, clinical psychology, developmental psychology, dietetics, early childhood education, education, elementary education, family/community counseling, human services, nutrition, nursing, occupational therapy, physical therapy, psychology, school psychology, social work, sociology, speech-language pathology or special education. A potential candidate may be a licensed registered nurse. Or potential candidate may have a related bachelor’s or master’s degree. Related is defined as at least 20 percent of the credit hours of transcript much address child development, disabilities, or family systems.
Service coordinator must have reliable transportation to travel within the 10 county area (Bleckley, Dodge, Johnson, Laurens, Montgomery, Pulaski, Telfair, Treutlen, Wheeler, and, Wilcox Counties).
